目的 分析Ki-67在乳腺癌病灶中的表达及其与18F-FDG PET-CT标准化摄取值和肿瘤本底比值的相关性。方法 选取乳腺浸润性导管癌患者50例,检测Ki-67在乳腺癌病灶中的表达及行PET-CT检查并计算乳腺癌病灶最大标准化摄取值和最大肿瘤本底比值。结果 50例患者中,36例(72%)Ki-67高表达,14例(28%)Ki-67低表达。与低表达组相比,高表达组患者最大标准化摄取值、最大肿瘤本底比值较高(P<0.001)。Pearson相关分析显示,Ki-67表达与18F-FDG PET-CT乳腺癌病灶最大标准化摄取值和最大肿瘤本底比值呈正相关(r=0.425,0.546;P=0.003,0.000)。最大标准化摄取值和最大肿瘤本底比值诊断Ki-67高表达的ROC曲线下面积分别为0.853、0.904。最大标准化摄取值最佳诊断界值为9.1,此时敏感性为62.3%,特异性为100%;最大肿瘤本底比值最佳诊断界值为13.5,此时敏感性为70.1%,特异性为100%。结论 最大肿瘤本底比值诊断乳腺癌病灶Ki-67高表达的价值比最大标准化摄取值高,能更好地反映治疗效果和判定预后。 |

Objective To analyze the expression of Ki-67 in mammary cancer and its correlation with standardized uptake value and tumor to background ratio of 18F-FDG PET-CT.Methods Fifty patients with invasive breast ductal carcinoma were selected for the detection of expression of Ki-67 and underwent PET-CT and maximum standardized uptake value and maximum tumor-to-background ratio were calculated.Results Among the 50 patients,Ki-67 was highly expressed in 36 (72.0%) and lowly expressed in 14 (28.0%).Compared with low expression group,patients in high expression group had higher maximum standardized uptake value and maximum tumor-to-background ratio (P<0.001).Pearson correlation analysis showed that Ki-67 expression was positively correlated with the maximum standardized uptake value and the maximum tumor-to-background ratio of breast cancer lesions (r=0.425,0.546;P=0.003,0.000).The area under ROC curve of maximum standardized uptake value and the maximum tumor-to-background ratio in diagnosis of high expression of Ki-67 were 0.853,0.904.The best diagnostic value of maximum standardized uptake value was 9.1;the sensitivity,specificity were 62.3%,100%;the best diagnostic value of maximum tumor-to-background ratio was 13.5;the sensitivity,specificity were 70.1%,100%.Conclusions The value of the maximum ratio of tumor-to-background ratio in the diagnosis of breast cancer with high expression of Ki-67 was higher than that of the maximum standardized uptake value and it can better reflect the therapeutic effect and determine the prognosis. |
