目的 探讨尿素氮、肌酐、尿酸联合检测在妊娠期高血压疾病肾损害评估中的应用。方法 采用日立7180全自动生化分析仪测定妊娠期高血压疾病患者132例[按24 h尿白蛋白排泄率(UAER)分为轻度46例、中度41例、重度45例]血清尿素氮、肌酐、尿酸水平。结果 妊娠期高血压疾病组尿素氮、肌酐、尿酸水平明显高于对照组(P<0.05)。妊娠期高血压疾病肾损害重度组血清尿素氮、肌酐、尿酸水平明显高于中度组和轻度组,而中度组血清尿素氮、肌酐、尿酸水平明显高于轻度组(P<0.05)。尿素氮、肌酐、尿酸为诊断指标的ROC曲线下面积为0.754(95%CI0.722~0.798)、0.801(95%CI0.715~0.869)和0.826(95%CI0.728~0.881)。结论 血清尿素氮、肌酐、尿酸水平在妊娠期高血压疾病患者中明显升高,并且随肾损害加重呈进行性升高,对妊娠期高血压疾病患者中重度肾损害具有较高的诊断价值。 |

Objective To explore the application of urea nitrogen,creatinine,uric acid combined detection in kidney damage assessment of pregnancy-induced hypertension.Methods Serum urea nitrogen,creatinine,uric acid were detected by Olympus AU 1000 automatic biochemical analyzer in 132 cases of pregnancy-induced hypertension [assigned into mild with 46 cases,moderate 41 cases,severe 45 cases according to 24h urinary albumin excretion rate (UAER)].Results Serum urea nitrogen,creatinine,uric acid in pregnancy-induced hypertension group were significantly higher than those in control group(P<0.05).Serum urea nitrogen,creatinine,uric acid in severe pregnancy-induced hypertension group were significantly higher than those in mild and moderate group(P<0.05),and those in moderate group were significantly higher than those in mild group(P<0.05).The areas under ROC curve of serum urea nitrogen,creatinine,uric acid were 0.754(95%CI 0.722~0.798),0.801(95%CI 0.715~0.869) and 0.826(95%CI 0.728~0.881),respectively.Conclusions Serum urea nitrogen,creatinine,uric acid are significantly increased in pregnancy-induced hypertension,and gradually increased with aggravation of kidney damage,which has high diagnostic value in assessment for moderate and severe kidney damage of pregnancy-induced hypertension. |
