      目的 探讨慢性阻塞性肺疾病(COPD)患者脂代谢和血清促血管生成素-2(ANG-2)水平与病情严重程度的关系。方法 选取慢性阻塞性肺疾病患者83例,同时选取体检的健康志愿者40例作为对照,比较不同缺氧程度、不同肺功能损害程度以及COPD合并肺心病患者的脂代谢指标和ANG-2水平与健康对照间的差异;分析COPD患者中脂代谢指标和ANG-2水平的相关关系。结果 COPD患者的总胆固醇(TC)和三酰甘油(TG)均低于健康对照组,COPD患者的内脂素和ANG-2均高于健康对照组,且差异有统计学差异(P<0.05);COPD患者的TC和TG水平随着缺氧程度和肺功能损害程度的增加而降低,内脂素和ANG-2水平随着缺氧程度和肺功能损害程度的增加而升高,且差异有统计学意义(P<0.05)。单纯COPD患者的TC和TG水平显著高于COPD合并肺心病的患者,单纯COPD患者的内脂素和ANG-2水平显著低于COPD合并肺心病的患者,且差异有统计学意义(P<0.05)。COPD患者的TC(r=-0.258,P=0.004)和TG(r=-0.240,P=0.007)水平与ANG-2水平呈负相关;COPD患者的内脂素水平与ANG-2水平呈正相关(r=0.476,P<0.001)。结论 脂代谢水平和ANG-2水平随着COPD患者的缺氧程度和肺功能损害程度而变化,且脂代谢水平和ANG-2水平密切相关。

      Objective To investigate the relationship between serum ANG-2 levels and lipid metabolism in patients with COPD.Methods We selected 83 patients with chronic obstructive pulmonary disease and 40 cases of healthy volunteers having physical examinations as a control to compare hypoxia,impairment of lung function,lipid metabolism as well as ANG-2 levels between patients of COPD with pulmonary heart disease and healthy controls.The correlation between lipid metabolism and ANG-2 levels in patients with COPD was also analyzed.Results Total cholesterol (TC) and triglyceride (TG) were significantly lower in patients with COPD than the healthy controls,while visfatin and ANG-2 levels were significantly higher in patients with COPD than the healthy controls(P<0.05).TC and TG levels in COPD patients decreased with the worsening of hypoxia and impairment of lung function,while visfatin and ANG-2 levels increased with the worsening of hypoxia and impairment of lung function,and the difference was statistically significant (P<0.05).TC and TG levels were significantly higher in COPD patients than in COPD patients with pulmonary heart disease,and visfatin and ANG-2 levels were significantly lower COPD patients than in COPD patients with pulmonary heart disease (P<0.05).TC (r=-0.258,P=0.004) and TG (r=-0.240,P=0.007) of COPD patients were negatively correlated with ANG-2 levels,while visfatin levels of COPD patients were positively correlated with ANG-2 levels(r=0.476,P<0.001).Conclusions Lipid metabolism and ANG-2 levels change with the degree of hypoxia and impairment of lung function in patients with COPD,and lipid metabolism is closely related with ANG-2 levels.
