| 目的 观察吡酮莫特治疗慢性鼻窦炎的疗效以及对血清白细胞介素-8(IL-8)和IL-13的影响。方法 将98例慢性鼻窦炎患者按数字表法随机分为对照组和观察组,每组49例；对照组进行如下治疗；鼻腔冲洗:采用0.9％氯化钠注射液500 mL冲洗鼻腔,每天2次。持续严重鼻塞者可使用0.5％呋嘛滴鼻液,每次2滴,每天3次,疗程＜7 d；口服克拉霉素片,每次0.25 g,每12 h 1次；观察组在对照组用药基础上口服匹多莫德颗粒,开始2周每次0.4 g,每天2次；后2周每次0.4 g,每天1次。两组均连续治疗4周。比较两组视觉模拟量表(VAS)评分、鼻窦CT Lund-Mackay评分、鼻内镜Lund-Kennedy评分；检测两组血清中IL-8和IL-13水平。结果 观察组治疗后2周和4周患者VAS评分、鼻窦CT Lund-Mackay评分以及鼻内镜Lund-Kennedy评分均明显少于同期对照组(P＜0.01)。观察组治疗后患者血清IL-8和IL-13水平均明显低于对照组,差异有统计学意义(P＜0.01)。结论 吡酮莫特治疗慢性鼻窦炎的疗效显著,抑制患者血清IL-8和IL-13水平可能为其作用机制之一。 |

| Objective To observe efficacy of pyrrone mulder in treatment of chronic nasosinusitis and its influence on interleukin-8(IL-8) and IL-13.Methods Totally 98 patients with chronic nasosinusitis treated in First Affiliated Hospital of Chengdu Medical College from February 2013 to February 2015 were randomized into control group and observation group with 49 cases in each group according to the random number table method.Control group was treated with 500 mL 0.9% sodium chloride injection for nasal douche twice a day.Patients with severe nasal obstruction were treated with 0.5% ephedrine nasal drops with 2 drops/time,tid and within 7 ds,together with oral intake of clarithromycin tablets,0.25g/time and 1time/12h.Observation group was given Pyrrone Mulder granule,0.4g/time and 2times/d for the first two weeks,0.4g/time and 1 time/d for the next two weeks.Courses were for 4 weeks in both groups.Scores of visual analogue scale,paranasal sinus CT Lund-Mackay,and nasal endoscopy Lund-Kennedy were compared between both groups.Expression levels of IL-8 and IL-13 in two groups were detected.Results After 2 and 4 weeks of treatment,scores of visual analogue scale,paranasal sinus CT Lund-Mackay,and nasal endoscopy Lund-Kennedy in observation group were obviously lower than those in control group(P＜0.01).Levels of IL-8 and IL-13 of observation group were remarkably lower than those of control group after treatment with statistic difference(P＜0.01).Conclusions Pyrrone mulder could be effective in treatment of chronic nasosinusitis,and inhibiting levels of IL-8 and IL-13 may be one of the mechanisms. |
