| 目的 探讨在无痛人流手术中,应用卡前列甲酯栓(卡孕栓)对女性宫颈软化、子宫收缩、手术时间、出血的影响。方法 将自愿实施无痛人流妊娠患者分为两组,观察组961例,术前30 min和术后阴道放置卡孕栓各0.5 mg,静脉注射麻醉药0.2 g丙泊酚；对照组221例,未予卡孕栓,经静脉注射麻醉药0.2 g丙泊酚,比较两组的宫颈软化程度、子宫收缩、手术时间、术中和术后出血、并发症和残留率等情况。结果 观察组中885例(92.09%)宫颈口可以通过6 号扩张棒在宫内操作,对照组仅69例(31.22%),观察组手术时间、术中出血量、术后阴道流血时间和残留率分别为(4.96±0.89)min、(14.86±6.83)mL、(6.04±1.27)d、12例(1.25%),对照组分别为(8.65±1.92)min、(20.34±8.62)mL、(9.37±1.82)d、11例(4.98%),与对照组比较,均差异有统计学意义(P<0.05或P<0.01)。结论 无痛人流术,术前使用卡孕栓可使术中子宫收缩良好,手术出血时间和出血量减少,降低了手术风险术后残留率,值得在临床中推广应用。 |

| Objective To study the effect of carboprost methylate suppositories (carboprost suppositories) on softening of cervix,uterine contraction,operation time,and hemorrhage in painless induced abortion.Methods Pregnant women who underwent painless induced abortion voluntarily in 2015 were randomized into two groups,the observation group (n=961) with respective 0.5 mg of carboprost suppositories placed vaginally30 min preoperatively and postoperatively and intravenous anesthetics 0.2 g of propofol,and the control group (n=221) with only intravenous anesthetics 0.2 g of propofol.Comparison was made between the two groups in cervical softening,uterine contraction,operation time,intraoperative and postoperative hemorrhage,complications and residual rate.Results In 885 cases (92.09%) of the observation group the No.6 cervical dilating rods could be used in the intrauterine operation,much more than 69 cases (31.22%) of the control group .In the observation group operation time,intraoperative blood loss,postoperative vaginal hemorrhage time and residual rate were (4.96±0.89) min,(14.86±6.83) mL,(6.04±1.27) d,12 cases (1.25%) ;compared with (8.65±1.92) min,(20.34±8.62) mL,(9.37±1.82) d,11 cases (4.98%) in the control group.There were statistically significant differences between the two groups (P< 0.05 or P< 0.01).Conclusions Preoperative use of carboprost suppositories for painless induced abortion can make the intraoperative uterine contract well,shorten operation time,decrease hemorrhage,and reduce the operation risk as well as postoperative residual rate,which is worthy of clinical application. |
