| 目的 探讨中药止血汤联合内镜下组织胶注射治疗胃底静脉曲张破裂出血的临床疗效。方法 选择住院的49例肝硬化门脉高压上消化道出血的患者,系统抽样随机分为观察组和对照组,经内镜下确诊均为胃底静脉曲张破裂出血。对照组21例患者仅采用西医及内镜下组织胶注射治疗；观察组28例患者在采用对照组的基础上加用止血汤治疗。并随访6个月,观察相关指标,比较两组患者的治疗效果。结果 经治疗,观察组显效20例,有效7例,无效1例,总有效率为96.4%。对照组显效12例,有效6例,无效3例,治疗总有效率为85.7%。观察组治疗效果显著优于对照组(P<0.05)；两组患者不良反应发生率、静脉曲张消失率及再出血率等方面差异有统计学意义(P<0.05)。结论 中药止血汤联合内镜组织胶注射治疗肝硬化门脉高压并发胃底静脉破裂出血可显著减少再出血率、门静脉血栓形成及不良反应,提高静脉曲张消失率,疗效显著,需临床进一步观察。 |

| Objective To observe the clinical curative effect of hemostatic decoction combined tissue adhesive injection(TAI) on gastric varicosis rupture haemorrhage.Methods Forty-nine cases were randomized into the treatment group of 28 cases and the control group of 21 cases.The control group was given western medicine and therapeutic endoscopy,while the treatment group was treated with modified hemostatic decoction and therapeutic endoscopy.They were followed up for six months by observing the relevant indicators and comparing the clinical effects of the two groups.Results The effective rate of the treatment group was 96.4%,27 cases markedly improved ,higher than 85.7% in the control group (P<0.05).There were significant differences in the incidence of adverse reactions,the disappearance rate of varicose veins and rebleeding rate(P<0.05).Conclusions Modified hemostatic decoction and therapeutic endoscopy treatment of portal hypertension complicated by gastric vein rupture hemorrhage can obviously reduce the rate of rebleeding,portal vein thrombosis and adverse reactions,and improve rate of varicose veins disappearance.Though it achieved distinct efficacy,it still needs to be further clinically observed. |
