      目的 从利益相关者角度出发,调查乡镇居民对医疗联合体(简称医联体)的知晓程度和期望,为卫生行政部门推动医疗联合体改革提供决策依据。方法 采用偶遇抽样方法对贵州省大方县长石镇、百纳乡,遵义县马蹄镇、平正乡,绥阳县小关乡,桐梓县天坪乡等乡镇地区居民进行问卷调查,总共调查104人,提取数据进行分类汇总。结果 受访居民以中老年人为主,居民对医联体的了解程度较低,72.1%的受访者表示首选就诊医疗机构为所在地乡镇卫生院,受访者中43.3%表示转诊不方便,45.2%表示未转诊过。受访者对医疗费用期望值最高的是“遵守收费政策,杜绝不合理收费”;对高效就医期望值最高的是“看病方便快捷”;对疗效期望值最高的是“医务人员具有较高的医疗技术和知识”、“符合诊疗规范”;对医疗服务最期望的是“医务人员平等对待患者”;对医疗辅助服务设施最期望的是“公示收费项目与标准”。结论 进一步推进医疗联合体改革需要加大宣传力度,提高基层医疗机构服务能力,简化双向转诊程序,树立以人为本的理念,积极建立医疗信息公开制度,有利于提高居民满意度及医联体的社会声誉。

      Objective To investigate the cognition and expectation about regional medical union(RMU) among the rural residents in Guizhou from the perspective of stakeholders and to provide a decision-making basis for promoting RMU reform.Methods Convenience sampling was adopted and 104 residents selected from 6 towns in Guizhou were surveyed by filling out self-made questionnaire,which included Changshi and Baina in Dafang county,Mati and Pingzheng in Zunyi county,Xiaoguan in Suiyang county and Tianping in Tongzi county.And then the data were collected and analyzed.Results Most interviewees were middle aged and older people,and they knew very little about RMU.Township hospitals were the first choice for 72.1% of interviewees,43.3% of interviewees complained about inconvenient referral,and 45.2% of interviewees said they never experienced referral.The highest expectations of interviewees were “following government charging policy and preventing unreasonable charges” in medical cost,“convenient and efficient treatment” in hospital process,“sophisticated medical technique” and “conforming to diagnosis criteria” in curative effect,“patients should be equally treated” in medical service,and “notified charging items and standards” in medical assistance.Conclusions It is necessary to strengthen publicity,to improve the service ability of basic medical institutions,to simplify bidirectional referral procedure,to cultivate the people-oriented idea and to establish a medicine information notification system.All of these contribute to the improvement of residents’ satisfaction and the social reputation of RMU.
