| 目的 探讨应用气腹针穿线悬吊阑尾配合双孔腹腔镜阑尾切除术(LA)的安全性、可行性、技术要点及优势。方法 回顾性分析应用气腹针穿线悬吊阑尾配合双孔腹腔镜阑尾切除术18例患者的临床资料。结果 18例均手术成功,手术时间23～76 min,平均(32±6)min。术中出血量为4～10 mL,患者术后肛门排气时间为6.5～20.5 h,平均(9.0±2.6)h。术后无肠瘘、切口感染、腹腔脓肿等近期并发症发生。术后切口微创美观。术后病理检查示急性化脓性阑尾炎5例、急性单纯性阑尾炎9例,慢性阑尾炎4例。全组患者获得随访(门诊随访7例、电话随访11例),术后平均随访时间为4个月(2～10个月),未发生戳孔疝、黏连性肠梗阻等远期并发症,手术瘢痕不明显。结论 应用气腹针穿线悬吊阑尾配合双孔腹腔镜阑尾切除术是安全可行的,气腹针穿线操作简单、经济实用,是一种简单易行的方法。 |

| Objective To explore the value(feasibility,safety,the main technical points and advantages)of pneumoperitoneum needle thread hanging and fastening appendix in double-hole laparoscopic appendectomy(LA).Methods The clinical data of 18 cases of appedndicitis who underwent improved laparoscopic appendectomy via hanging and fastening appendix with pneumoperitoneum needle thread were analyzed retrospectively.Results All 18 cases achievedsuccessful operation.The operation time was averaged(32±6)min(23~76 min).The blood loss during operation was 4~10 mL.The anus exhaust time was averaged(9.0±2.6)h(6.5~20.5 h).There were no complications including intestinal fistula,incision infection,abdominal abscess.All patients were discharged and abdominal incision hidden.After 2~10 months of follow-up,there were no complications including intestinal fistula or obstruction,incision infection,bleeding,abdominal abscess.Incision healed well and achieved good cosmetic effect in all patients.Conclusions Themethod is safe,feasibleand simple.There are many advantages including hidden incision,good cosmetic effect and lower cost,so it is worthy of clinical application. |
