| 目的 探讨葡萄糖酸钙联合卡前列素氨丁三醇预防高危产妇剖宫产产后出血的临床疗效,为临床治疗方案的选择提供理论依据。方法 选取行剖宫产手术的135例高危产妇,其中45例在胎儿娩出后采用缩宫素静脉滴注(A组),45例采用缩宫素静注联合卡前列素氨丁三醇肌注(B组),45例在前两组基础上给予在术前葡萄糖酸钙静脉滴注(C组),比较三组子宫收缩情况,产后2 h内和2 h后的出血量及产后出血发生率,并观察不良反应发生情况。结果 子宫收缩优良率从低到高分别为A组、B组和C组(P<0.01)；产后2 h内出血量从低到高分别为C组、B组和A组(P<0.01)；2 h后出血量三组差异无统计学意义(P>0.05)。结论 葡萄糖酸钙联合卡前列素氨丁三醇在预防高危产妇产后出血中可有效降低产后出血发生率和出血量,改善子宫缩复功能,不良反应低,具有较高的临床疗效和安全性。 |

| Objective To investigate the preventive effect of calcium gluconate combined withcarboprosttromethamineon postpartum hemorrhage of high-risk cesarean puerpera,and to provide a theoretical basis for clinical treatment.Methods A hundred and thirty-five high-risk women undergoing obstetric cesarean section from May 2014 to December 2015 in Worker′s Hospital of Qinhuangdao city were enrolled in the study,of which 45 were treated with intravenous oxytocinafter delivery(group A),45 caseswith oxytocin combined withcarboprosttromethamine(group B),and another 45 cases with additional preoperative calcium gluconate infusion(group C).The uterine contraction,the postpartum bleeding volume within and after 2 hours,the incidence of postpartum hemorrhage and the adverse events were observed and compared amongthe three groups.Results The excellent rates of uterine contraction ranking from low to high were group A,group Band group C(P<0.01);2-hour postpartum hemorrhage ranking from low to high were group C,group Band group A(P<0.01);bleedingafter 2 hours had no significant difference amongthe three groups(P>0.05).Conclusions Calcium gluconatecombined withcarboprosttromethamine in preventing high-risk postpartum hemorrhage can effectively reduce the incidence of postpartum hemorrhage and bleeding,improve uterine contractions,and reduceadverse reactions,which achieve better clinical efficacy and safety. |
