| 目的 探讨临床药师对临床不合理用药医嘱进行事先干预的方法和成效。方法 临床药师对住院用药医嘱进行审核,对不合理的用药医嘱进行干预。分析全年数据,对不合理用药的干预方式、数量、类型、科室分布、药品种类、医生采纳率、被干预药品的用量变化等进行评价。结果 全年共干预不合理住院用药医嘱共718条,有效采纳率为92.5%；不合理用药的主要类型有:无指征或超适应证用药、重复用药、用法用量及疗程过长等问题；前10位被干预药品的用量下降10%~40%。结论 临床药师对住院不合理用药医嘱的事先干预有显著效果,比事后点评更有实际的效用。此方法可将不合理用药防患于未然,被干预药品的不合理情况及用量明显下降；同时也锻炼和提高了临床药师的用药技能水平。 |

| Objective To explore the methods and effects of exante intervention of irrational drug use by clinical pharmacists.Methods Clinical pharmacists audited medication orders and intervene in the irrational drug use.Data in the whole year were analyzed.The intervention methods,quantities,types,distribution of departments,drug types,the adoption rate of the doctors,and dose change of the irrationally used drugs were evaluated.Results In 2015,a total of 718 irrational drug use were intervened by clinical pharmacists,and the effective adoption rate was 92.5%.The main types of irrational use of drugs were no indication or excessive indication of drug use,repeated use,overdosage,excessive course of treatment and so on.The amount of the first ten drugs was decreased by 10%~40%.Conclusions The exante intervention of irrational drug use by clinical pharmacists had a better result than prescription reviews,which could prevent irrational drug use in advance.The amount of irrational drug use was obviously decreased.Meanwhile,the medication skills of clinical pharmacists were improved. |
