| 目的 探讨硝苯地平联合硫酸镁对妊娠期高血压疾病母婴的影响。方法 选取妊娠期高血压疾病患者240例,按随机数字表法随机分为联合组及对照组,每组120例。联合组患者采用硝苯地平联合硫酸镁治疗,对照组采用单用硫酸镁治疗,对比两组患者治疗后母婴情况、疗效及并发症的发生情况。结果 与对照组相比,联合组患者治疗后宫缩乏力、被迫终止妊娠、产后大出血、胎儿宫内窘迫、生后窒息、围生期死亡发生率较低(P<0.05)。联合组患者总有效率为92.5%,高于对照组(75.8%),差异有统计学意义(Z=12.507,P<0.001)。与治疗前相比,联合组与对照组患者治疗后收缩压、舒张压、24 h尿蛋白、血液黏滞度、纤维蛋白原较低,凝血酶原时间较长(P<0.05)。与对照组相比,联合组患者治疗后收缩压、舒张压、24 h尿蛋白、血液黏滞度、纤维蛋白原较低,凝血酶原时间较长(P<0.001)。结论 硝苯地平联合硫酸镁治疗妊娠期高血压疾病疗效佳,并发症少,预后可,值得临床推广。 |

| Objective To explore the influence of nifedipine combined with magnesium sulfate on edema-hypertension-proteinuria syndrome.Methods Totally 240 patients with edema-hypertension-proteinuria syndrome treated in The Seventh People′s Hospital affiliated to Shanghai University of Traditional Chinese Medicine were randomized into combined group and control group according to stochastic indicator method.And every group had 120 patients.Patients in the combined group were treated by nifedipine combined with magnesium sulfate while patients in the control group were treated by magnesium sulfate.Maternal and infant condition,curative effect and complication of patients in two groups were compared.Results Compared with control group,patients in the combined group had lower rates of uterine atony,termination of pregnancy,postpartum hemorrhage,fetal distress,perinatal asphyxia,perinatal death after treatment (P<0.05).The total effective rate of patients in combined group was 92.5%,higher than that in control group (75.8%) (Z=12.507,P<0.001).Compared with patients before treatment,patients in the combined group and control group had lower systolic blood pressure,diastolic blood pressure,24 h urinary protein,blood viscosity,fibrinogen and longer prothrombin time (P<0.05).Compared with control group,patients in the combined group had lower systolic blood pressure,diastolic blood pressure,24 h urinary protein,blood viscosity,fibrinogen and longer prothrombin time (P<0.001).Conclusions Nifedipine combined with magnesium sulfate in the treatment of edema-hypertension-proteinuria syndrome has good curative effect,fewer complications,good prognosis,which is worthy of clinical promotion. |
