| 目的 探讨新生儿先天性膈疝(CDH)伴动脉导管未闭(PDA)联合手术的围术期护理。方法 回顾分析3例新生儿CDH伴PDA患儿的临床资料。术前积极改善呼吸循环功能,给予禁食水、胃肠减压、静脉营养、纠正酸碱紊乱等维持内环境稳定,做好与多部门的沟通、协调工作,待情况稳定后进行手术；术后加强生命体征监测、呼吸机的使用及呼吸道的管理、循环功能维护、营养支持等,防范各种并发症的发生。结果 治愈 2 例,另1例因术后出现膈疝复发家长放弃治疗。结论 新生儿CDH伴PDA病情危重,联合手术挽救了患儿的生命,减少患儿二次手术的损伤,但是对医护团体要求较高,加强围术期护理是手术成功的有效保障。 |

| Objective To explore the perioperative nursing of joint surgery combining congenital diaphragmatic hernia(CDH) with patent ductus arteriosus (PDA) in neonates.Methods Clinical data were collected from 3 cases of neonates of CDH combined with PDA.We thought that actively improving the respiratory and circulation function,giving fasting and water deprivation,gastrointestinal decompression,intravenous nutrition,correcting acid-base disturbances was necessary preoperatively to maintain a stable internal environment.It was also important to have good communication and coordination with multiple departments so that the operation could be performed after the respiration and circulation had been stabilized.We should strengthen the monitoring of patients′ vital signs,good management of ventilator and nutritional support to prevent complications after surgery.Results Two patients were cured,and another 1 case quit therapy because of the recurrence of postoperative diaphragmatic hernia.Conclusions Neonates of CDH combined with PDA were in critical condition.Joint surgery could save the patient's life and reduce the injury of the second surgery but with higher demand of the medical group.To strengthen perioperative nursing is the effective guarantee of a successful operation. |
