| 目的 探讨人工全膝置换(TKA)术中髌下脂肪垫的保留与切除对术后膝关节功能及髌骨高度的影响。方法 对56例骨关节炎病人(72个膝关节)采用人工全膝置换术,分为A、B两组,A组为保留髌下脂肪垫(36个膝关节),B组为切除髌下脂肪垫(36个膝关节),对两组病人术前和术后的膝关节活动度、HSS膝关节功能评分及Insall-Salvati比值进行统计学分析。结果 术后随访2年,A、B两组膝关节活动度及HSS膝关节功能评分差异无统计学意义(P>0.05),A组术前及术后的Insall-Salvati比值差异无统计学意义(P>0.05),而B组术前及术后的Insall-Salvati比值差异有统计学意义(P＜0.05)。结论 TKA术中髌下脂肪垫的保留与切除对术后膝关节功能无明显影响,而髌下脂肪垫的切除可导致低位髌骨。 |

| Objective To investigate the artificial total knee arthroplasty(TKA) surgery in the infrapatellar fat pad retention and resection on the effect of postoperative knee function and patellar height. Methods 56 cases of osteoarthritis patients(72 knees) with artificial total knee arthroplasty in our hospital were divided into two groups:group Aand group B.Group Awere retain the infrapatellar fat pad(36 knees) while group Bfor the resection of infrapatellar fat pad(36 knees).The two groups of patients with preoperative and postoperative range of motion of the knee,HSS knee function score and Insall-Salvati ratio were statistically analyzed. Results After 2 years of follow-up,A,B two groups of knee joint mobility and HSS knee joint function score difference has no statistical significance(P>0.05).A group of preoperative and postoperative Insall-Salvati ratio difference has no statistical significance(P>0.05),while group Bpreoperative and postoperative Insall-Salvati ratio difference was statistically significant(P<0.05). Conclusion Retain TKA intraoperative patellar fat pad under the resection has no obvious effect on postoperative function of knee joint,and the patellar resection can lead to low patella. |
