| 目的 研究Castleman病的临床特点、组织病理、治疗及预后。方法 回顾性分析安徽省立医院病理科2003年1月—2016年6月诊断的Castleman病45例。结果 45例Castleman病中,透明血管型 39例,浆细胞型6例。39例透明血管型滤泡生发中心可见嗜伊红无结构基质,小淋巴细胞围绕成层状“洋葱皮”样结构；滤泡间毛细血管增生可见血管植入生发中心,形成“棒棒糖”结构,6例浆细胞型的生发中心萎缩及套区增宽不明显,无典型的洋葱皮样结构及血管植入现象,滤泡间大量浆细胞浸润,可见毛细血管增生,无明显玻璃样变性。29例获得完整随访资料,随访时间为4～127个月。结论 Castleman病是一种少见的慢性淋巴组织增生性病变,病因尚不明确,预后较佳；该组研究中,未获得死亡病例的记录。 |

| Objective To explore the clinical features,histopathology,therapy and prognosis of Castleman disease (CD).Methods A retrospective analysis was conducted of forty-five cases of castleman disease diagnosed in department of pathology,anhui provincial hospital from January 2003 to June 2016. Results These clinical cases included 39 cases of hyaline-vascular types,and 6 cases of plasma cell types.Histopathologically,in hyaline-vascular types of Castleman disease,the follicle germinal centers exhibited eosinophilic unstructured matrix,and small lymphocytes formed in an"onion skin" structure; interfollicular capillary proliferation was implanted into germinal centers and formed in a "lollipop" structure.In plasma cell types of Castleman disease,there were atrophic germinal centers and not obviously broadened mantle zone,without typical "onion skin" structure and vascular implantation.Abundant plasmacyte infiltration could be found in interfollicular area with the capillary proliferation.There was no obvious hyaline degeneration.Complete histological follow-up data were acquired in 29 cases with the follow-up period from 4 months to 127 months.Conclusions Castleman disease is a rare chronic lymphatic hyperplasia disease.Its pathogenesis is unknown yet the prognosis is good.No record of dead case has been found. |
