      目的 探讨甲泼尼龙对儿童难治性肺炎支原体肺炎(RMPP)的疗效及安全性。 方法 检索PubMed、Ovid、ELSEVIER ScienceDirect(SDOS)数据库、中国生物医学文献数据库、中国知网和万方数据库,检索的起止时间为建库至2016年10月31日。根据纳入和排除标准筛选文献、资料,获得甲泼尼龙治疗儿童RMPP的随机对照试验(RCT)文献,进行文献质量评估后用Renman5.3软件进行Meta分析。 结果 共检索到相关文献904篇,最终纳入29篇RCT文献进行Meta分析。Meta分析结果提示甲泼尼龙治疗儿童RMPP有效,可明显缩短住院时间(MD=-4.05,95%CI:-4.09~-3.20,P<0.000 01),血清C-反应蛋白(CRP)水平亦明显低于对照组(MD=-23.21,95%CI:-31.63~-14.79,P<0.000 01),并能明显缩短体温恢复时间(MD=-3.12,95%CI:-3.57~-2.67,P<0.000 01)、肺部啰音消失时间(MD=-3.56,95%CI:-4.40~-2.73,P<0.000 01))、咳嗽消失时间(MD=-3.43,95%CI:-4.27~-2.59,P<0.000 01)),提高临床有效率(OR=5.11,95%CI:3.58~7.31),P<0.000 01),仅有少数轻微不良反应。 结论 在常规治疗基础上加用甲泼尼龙治疗儿童RMPP疗效显著,且安全性较高。

      Objective To explore the efficacy and safety of methylprednisolone in adjuvant treatment of children with refractory mycoplasma pneumoniae pneumonia (RMPP). Methods We retrieved the PubMed,Ovid,ELSEVIER ScienceDirect (SDOS),CBM,CNKI and Wanfang database from the establishment of the database to October 2016.Randomized controlled trials (RCTs) about using methylprednisolone as an active intervention in children with RMPP were selected,screened by inclusion and exclusion criteria.The screened literatures were evaluated and Meta analyzed by Revman 5.3 software. Results A total of 904 relevant documents were retrieved,29 RCTs selected for Meta-analysis.The results of the Meta-analysis showed that methylprednisolone in the treatment of RMPP was efficient and significantly shortened the time of hospitalization [MD=-3.83,95%CI:(-4.51)-(-3.41),P<0.000 01],reduced serum levels of CRP [MD=-23.21,95%CI:(-31.63)-(-14.79),P<0.000 01],shortened the disappearance time of hyperpyrexia [MD=-3.12,95%CI:(-3.57)-(-2.67),P<0.000 01],lung rale [MD=-3.56,95%CI:(-4.40)-(-2.73),P<0.000 01] and cough [MD=-3.43,95%CI:(-4.27)-(-2.59),P<0.000 01];the total efficiency was significantly higher than that of the control group [OR=5.11,95%CI:3.58-7.31,P<0.000 01].A few slight adverse reactions of methylprednisolone were reported. Conclusions Methylprednisolone in the treatment of RMPP is convenient and safe.
