目的 观察并分析老年慢性阻塞性肺疾病(COPD)稳定期合并冠心病患者营养风险及相关营养指标白蛋白、前白蛋白及视黄醇蛋白水平与心功能分级的关系。方法 选取2014年1月至2017年1月安徽医科大学附属安庆医院门诊及住院治疗的老年COPD稳定期合并冠心病患者28例作为观察组,单纯老年COPD稳定期患者30例作为对照组。应用营养风险筛查量表2002(NRS-2002)进行营养筛查评分,并统计心功能分级情况。采用前瞻性研究的方法,对比观察两组的营养风险及白蛋白、前白蛋白、视黄醇蛋白水平营养相关指标,并分析营养风险与心功能分级的关系。结果 观察组患者轻度营养风险、中度营养风险和重度营养风险患者分别为16、6、3例,共计25例;对照组分别为15、3、1例,共计19例;观察组白蛋白(34.26±5.22) g·L-1,前白蛋白(20.60±7.03) mg·L-1,视黄醇蛋白(22.80±5.70) ng·L-1,均低于对照组。营养风险与心功能分级相关系数r=0.469,P=0.018。结论 老年COPD稳定期合并冠心病患者营养风险的发生率较高,且患者心功能水平分级与营养风险筛查评分呈正相关。临床上对于老年COPD稳定期合并冠心病患者应积极进行营养风险筛查,并早期进行合理的营养干预。 |

Objective To observe and analyze the nutritional risk and the relationship between albumin,prealbumin and retinol protein levels and cardiac functional grading in elderly patients with stable chronic obstructive pulmonary disease with coronary heart disease.Methods A prospective study was conducted to select 28 patients with stable chronic obstructive pulmonary disease complicated with coronary heart disease from January 2014 to January 2017 in our hospital as the observation group.Thirty patients with stable chronic obstructive pulmonary disease were treated as control group.Nutritional risk screening scale 2002(NRS-2002) was used for nutrition screening and scores.A prospective study was used to compare the nutrition risk of the two groups with the nutrition-related indicators of albumin,prealbumin,and retinol levels,and to analyze the relationship between nutritional risk and cardiac functional grading.Results There were 16 cases,6 cases,and 3 cases of mild,moderate,and severe nutrition risk in COPD and CHD patients,totaling 25 cases.The COPD group consisted of 15 cases,3 cases,and 1 case respectively.The albumin-related albumin of the observation group was(34.26±5.22) g·L-1,prealbumin(20.60±7.03) mg·L-1,retinal protein(22.80±5.70) ng·L-1,both lower than the control group.The correlation coefficient between nutrition risk and grade of heart function was r=0.469,P=0.018.Conclusion The prevalence of nutritional risk is higher in patients with COPD and coronary heart disease,and there is a positive correlation between the nutritional risk screening scores and the level of cardiac function.So for the patients with COPD in the elderly with coronary heart disease,clinical screening for nutritional risk should be actively conducted and reasonable nutrition intervention should be conducted early. |
