| 目的 探讨单极电刀在腹腔镜全子宫切除术(TLH)中对阴道残端愈合的影响。方法 选取2015年10月至2017年2月期间台山市人民医院收治的135例需行TLH治疗的病人,采用随机数字表法将病人分为单极电钩电切组、单极电钩电凝组和剪刀组,比较不同手术方法中病人手术指标、术后情况和术后随访情况。结果 单极电钩电切组和单极电钩电凝组分别与剪刀组病人手术指标进行比较,前两者的术中出血量、术后排气时间及术后住院时间较剪刀组降低,且差异有统计学意义(P＜0.05)。手术时间方面,三组均差异无统计学意义(P＞0.05)。单极电钩电切组和单极电钩电凝组分别与剪刀组病人术后情况进行比较,单极电钩电切组各项指标的发生率无明显升高,差异无统计学意义(P＞0.05)；单极电钩电凝组与剪刀组相比发生率明显升高,差异有统计学意义(P＜0.05)。单极电钩电切组和单极电钩电凝组分别与剪刀组病人术后随访情况进行比较,三者的体质量指数分别为(11.32±6.33)、(11.92±5.93)、(12.42±6.29)kg/m2,差异无统计学意义(P＞0.05)；更年期综合征发生率分别为22.22%、25.58%、23.40%,差异无统计学意义(P＞0.05)；尿促卵泡成熟素值分别为(8.66±6.53)、(9.34±6.17)、(10.12±5.86)U/24 h,差异无统计学意义(P＞0.05)。结论 对于需要行全子宫切除术的病人而言,单极电钩电切组、单极电钩电凝组和剪刀组在手术中取得的效果相当；而对于阴道残端愈合方面,单极电钩电切法能取得更好的效果。 |

| Objective To explore the effect of monopolar electrosurgical excision procedure on the healing of vaginal stump during total laparoscopic hysterectomy (TLH).Methods One hundred and thirty-five TLH patients in Taishan People's Hospital from October 2015 to February 2017 were studied,and the treated patients were randomly assigned into the monopolar electric hook electrotomy group,the monopolar electric hook electrocoagulation group and the scissor group by using the random number table method.The surgical indexes,postoperative conditions and postoperative follow-up of the patients undergoing different surgical procedures were compared.Results Compared with the scissor group,the monopolar electric hook electrotomy group and the monopolar electric hook electrocoagulation group had decreased blood loss,postoperative exhaust time and hospitalization time,and the difference was statistically significant (P <0.05).There was no significant difference in operation time among three groups (P＞0.05).Compared with the scissor group,the incidence of various indexes was not significantly increased in the monopolar electric hook electrotomy group;the difference was not statistically significant (P＞0.05).Compared with the scissor group,the incidence of various indexes was significantly increased in the monopolar electric hook electrocoagulation group,and the difference was statistically significant (P＜0.05).As for the follow-up outcomes among the monopolar electric hook electrotomy group,the monopolar electric hook electrocoagulation group and the scissor group,the body mass indexes (BMI) were (11.32±6.33),(11.92±5.93),and (12.42±6.29)kg/m2,respectively,and the difference was not statistically significant (all P>0.05);the climacteric syndrome rates were 22.22%,25.58%,and 23.40%,respectively,and the difference was not statistically significant (all P>0.05);and the values of follicle-stimulating hormone (FSH) were (8.66±6.53),(9.34±6.17),(10.12±5.86) U/24 h,respectively,and the differences were not statistically significant (all P>0.05).Conclusions As for patients undergoing total laparoscopic hysterectomy,the operative outcomes were equivalent among three groups.When it comes to vaginal stump healing,the monopolar electric hook electrotomy may achieve better results. |
