| 目的 探讨细胞角蛋白5/6(cytokeratin5/6,CK5/6)、P63蛋白、甲状腺转录因子-1(thyroid transcription factor,TTF-1)、细胞角蛋白7(cytokeratin7,CK7)在肺癌中的表达情况及临床意义。方法 收集潍坊医学院附属医院2016年1月至2017年4月经皮肺穿刺活检及经纤维支气管镜活检、刷检确诊肺癌病人标本110例,其中鳞癌48例,腺癌60例,腺鳞癌2例,通过免疫组化方法检测CK5/6、P63、TTF-1、CK7在不同病理类型中的表达情况。结果 CK5/6和P63在鳞癌中的阳性率分别是93.75%和93.75%,而在腺癌中的阳性率分别是5.00%和18.33%;TTF-1和CK7在腺癌中的阳性率分别是90.00%和93.33%,而在鳞癌中的阳性率是8.33%和25.00%(P＜0.05)。CK5/6、P63蛋白对肺癌中鳞癌诊断的灵敏度和特异度分别为93.75%、93.75%和95.00%、81.67%;而TTF-1,CK7检测对肺癌中腺癌诊断的灵敏度和特异度分别为90.00%、93.30%和91.67%、75.00%。结论 CK5/6,P63,TTF-1和CK7对肺癌活检标本中腺癌和鳞状细胞癌的鉴别具有诊断意义,可指导临床治疗。 |

| Objective To investigate the expression of cytokeratin 5/6 (cytokeratin 5/6,CK5/6),P63 protein,thyroid transcription factor-1 (TTF-1) and cytokeratin 7 (CK7) in lung cancer and clinical significance.Methods 110 cases from Weifang Medical College Affiliated Hospital during January 2016 to April 2017 were collected.All specimens were diagnosed with lung cancer by percutaneous lung biopsy,transbronchoscopic lung biopsy and brush biopsy by fiberoptic bronchoscope.Specimens included 48 cases of squamous cell carcinoma,60 cases of adenocarcinoma,2 cases of adenosquamous carcinoma.The expression of CK5/6,P63,TTF-1 and CK7 in different pathological types was detected by immunohistochemical technique.Results The positive rates of CK5/6 and P63 in squamous cell carcinoma were 93.75% and 93.75%,while the positive rates in adenocarcinoma were 5.00% and 18.33%.The positive rates of TTF-1 and CK7 in adenocarcinoma were 90.00% and 93.33%,and the positive rate in squamous cell carcinoma was 8.33% and 25.00% (P＜0.05).The sensitivity and specificity of CK5/6 and P63 protein in the diagnois of squamous cell carcinoma were 93.75%,93.75% and 95.00%,81.67%.And the sensitivity and specificity of TTF-1 and CK7 in the diagnosis of adenocarcinoma in lung cancer were 90.00%,93.30% and 91.67% and 75.00% respectively.Conclusion CK5/6,P63,TTF-1 and CK7 have definite significance in the identification of adenocarcinoma and squamous cell carcinoma in lung cancer biopsy specimens,which can guide clinical treatment. |
