| 目的分析血液净化联合乌司他丁对脓毒症急性肺损伤病人肺功能的改善效果及对病人基质金属蛋白酶及其抑制剂(MMPs/TIMPs)水平的影响，为临床治疗提供参考。方法上海市杨浦区市东医院 2017年 1月至 2018年月收治的脓毒症急性肺损伤 150例，采用随机数字表法分为对照组(n＝70)和观察组(n＝80)。两组研究对象入院时均进行常规的抗感染、器官功能保护、维持水、电解质、酸碱平衡和营养支持，对照组给予持续性血液净化，观察组给予持续性血液净化联合乌司他丁治疗 10 d，每次注射 30万单位，每日 3次。分析两组病人接受治疗前后肺功能的改善情况及血清中MMPs/TIMPs水平的变化。结对照组和观察组病人随着治疗时间的延长，肺功能指数肺活量［对照组和观察组病人治疗前 /治疗 10 d后分别为(1.27± 0.2果7)L/(1.71±0.65)L和(1.26±0.33)L/(1.90±0.44)L］、氧合指数［对照组和观察组病人治疗前 /治疗 10 d后分别为(225.5± 23.8)mmHg/(271.7±34.6)mmHg和(222.5±38.7)mmHg /(291.5±43.2)mmHg］及最大通气量［ MVV，对照组和观察组病人治疗前 /治疗 10 d后分别为(33.3±4.3)L/min /(43.8±5.4)L/min和(33.2±6.7)L/min /(49.3±5.1)L/min］均明显升高(P＜0.05)，急性生理学与慢性健康状况评分 Ⅱ(APACHE Ⅱ)及序贯器官衰竭评分(SOFA)评分均明显降低(P＜0.05)，且治疗相同时间点时观察组病人的上述指标比对照组病人变化更显著(P＜0.05)；两组病人经治疗后基质金属蛋白酶 2/9(MMP2/9)及基质金属蛋白酶抑制剂 1/2(TIMP1/2)水平均明显降低(P＜0.05)，且观察组病人的上述指标降低更明显(P＜0.05)。结论血液净化联合乌司他丁能有效改善脓毒症急性肺损伤病人肺功能、降低血清中基质金属蛋白酶及其抑制剂水平。 |

| Objective To explore the effects of blood purification combined with ulinastatin on pulmonary function and the level ofmatrix metalloproteinases and their inhibitors in patients with sepsis?induced acute lung injury.
