| 目的分析开放手术与电视胸腔镜辅助手术治疗创伤凝固性血胸的效果比较。方法选取黄冈市中心医院在 2015年 6月至 2016年 6月之间收治的创伤凝固性血胸病人 100例，根据随机数字表法均分为两组，其中观察组 50例应用电视胸腔镜辅助手术治疗，对照组 50例应用传统开放手术方式治疗，比较两组病人的临床治疗效果。结果观察组病人手术时间( 55.3±6.8)min、术中出血量( 103.6±2.1)mL、术后引流量( 212.6±9.1)mL、住院时间( 8.2±0.9)d，明显优于对照组的( 96.4±7.6)min、(187.7±3.5)mL、(369.7±11.2)mL、(14.6±1.3)d，两组比较差异有统计学意义( t＝28.50，145.70，76.98，26.83，P＜0.05)。观察组病人治愈率 86.0%，明显高于对照组的 64.0%，两组比较差异有统计学意义( χ2＝6.45，P＝0.01)；观察组病人治疗无效率及术后并发症发生率均为 2.0%，明显的低于对照组病人的 18.0%、16.0%，两组比较差异有统计学意义( χ2＝4.40，P＝0.04)。结论电视胸腔镜辅助手术治疗创伤凝固性血胸相比较于常规开放性手术治疗具有手术时间短，术中以及术后出血引流量少等优点，可以明显缩短病人的住院时间，提高病人的临床治疗有效率以及治愈率，具有较好的临床应用价值以及推广意义。 |

| Objective To explore and analyze the effect of open surgery and video?assisted thoracoscopic surgery in treatment oftraumatic clotted hemothorax.Methods 100 patients with traumatic coagulopathy who were admitted to Huanggang Central Hospi?tal from June 2015 to June 2016 were selected.According to the method of random number，the patients were divided into twogroups.50 patients in the observation group were treated with video?assisted thoracoscopic surgery，and 50 patients in the controlgroup were treated with traditional open surgery.The clinical effects of the two groups were compared.Results Operative time，in? traoperative blood loss，postoperative drainage volume in the observation group were(55.3±6.8)min，(103.6±2.1)mL，(212.6±9.1) mL，(8.2±0.9)d，significantly better than(96.4±7.6)min，(187.7±3.5)mL，(369.7±11.2)mL，(14.6±1.3)d in the control group，the difference between the two groups was statistically significant(t＝28.50，145.70，76.98，26.83，P＜0.05).The cure rate of the obser? vation group was 86.0%，significantly higher than 64.0% in the control group，the difference between the two groups was statistical? ly significant(χ2＝6.45，P＝0.01)； Ineffective treatment rate and postoperative complication rate of the observation group were 2.0%，significantly lower than 18.0%，16.0% of patients in the control group，the difference between the two groups was statistically significant(χ2＝4.40，P＝0.04).Conclusion Video assisted thoracoscopic surgery in the treatment of traumatic clotted hemotho? rax，with the advantages of short operation time and less bleeding after operation compared with conventional open surgery，can sig? nificantly shorten the average length of hospital stay and improve the clinical efficacy and cure rate，which has good clinical appli?cation value and popularization significance，and is worthy of clinical application. |
