      目的探讨成年男性内脏脂肪面积(VFA)与腰椎骨密度(BMD)的相关性。方法收集 2017年 5月至 2017年 8月入华北石油总医院体检男性共 1368例,年龄(50.91±0.30)岁,年龄范围为 25~65岁。所有研究对象均测定身高、体质量、腰围,计算体质指数(BMI);利用定量 CT(QCT)测量 L1、2椎体的平均骨密度值及 L2层面的腹部皮下脂肪面积(SFA)与 VFA。根据研究对象的骨密度分为骨量正常骨量组、低骨量组及骨质疏松组,比较各组年龄、 BMI、VFA、SFA的差异,并分析其与腰椎骨密度的相关性。结果正常骨量组 805例、低骨量组 449例及骨质疏松组 114例。正常骨量组、低骨量组和骨质疏松组年龄分别为(39.25±9.33)、(56.17±7.61)、(67.28±8.19)岁, BMI分别为(24.31±4.62)、(22.65±2.81)和(19.84±3.11)kg/m2;低骨量组和骨质疏松组年龄均高于正常骨量组,骨质疏松组年龄高于低骨量组;低骨量组和骨质疏松组 BMI均低于正常骨量组;骨质疏松组 BMI低于低骨量组;低骨量组和骨质疏松组内脏脂肪量(VAT)[(25.59±2.11)g和(39.68±5.05)g]和 VFA值均高于正常骨量组(24.20±2.09)g,同时骨质疏松组 VAT、VFA值明显高于低骨量组,均差异有统计学意义(P<0.05);男性腰椎骨密度与 VFA呈负相关,且控制年龄及 BMI因素后,男性腰椎骨密度与 VFA仍呈负相关。结论男性腰椎骨密度与内脏脂肪组织呈负相关关系。

      Objective To investigate the correlation between visceral fat area and lumbar bone mineral density in adult males. Methods A total of 1368 males aged 25 to 65 with an average age of(50.91±0.30)years were enrolled in North China Petro? leum General Hospital from May 2017 to August 2017.Height,weight and waist circumference were measured and body mass index(BMI)was calculated.Average BMD values of L1 and L2 vertebrae and subcutaneous fat area(SFA)and visceral fat area(VFA) at L2 level were measured by QCT.According to the BMD of the subjects,they were divided into normal bone mass group,low bone mass group and osteoporosis group.The differences of age,BMI,VFA and SFA among groups were compared,and the correlation be? tween them and lumbar BMD was analyzed.Results There were 805 people in normal bone mass group,449 in low bone mass group and 114 in osteoporosis group.The age of normal bone mass group,low bone mass group and osteoporosis group was(39.25±9.33),(56.17±7.61)and(67.28±8.19)years,respectively,and the BMIof normal bone mass group,low bone mass group and osteopo? rosis group was(24.31±4.62),(22.65±2.81)and(19.84±3.11)kg/m2.The VAT of normal bone mass group,low bone mass group and osteoporosis group was(24.20±2.09)g,(25.59±2.11)g and(39.68±5.05)g,respectivelythe VAT and VFA of the low bone mass group were higher than those of the normal group,and BMI was lower than that of the normal group.The age and VFA of osteoporo? sis group were higher than that of normal group,and BMI was lower than that of normal group.The age of osteoporosis group washigher than that of low bone mass group and BMI was lower than that of low bone mass group.The differences were statistically sig?nificant(P<0.05).BMD was negatively correlated with VFA in male lumbar spine,and it was negatively correlated with VFA in male lumbar spine after controlling age and BMI factors.Conclusion There is a negative correlation between bone mineral density and visceral fat tissue in males.
