      目的研究血清 S100钙结合蛋白 12(S100A12)与急性非静脉曲张性上消化道出血严重程度的相关性。方法选取 2017年 2月至 2018年 8月就诊于潍坊医学院附属医院经急诊胃镜诊断明确并收治入院的急性非静脉曲张性上消化道出血病人 90例。根据急性非静脉曲张性上消化道出血严重程度进行分级,分为轻度(n=27)、中度(n=42)、重度(n=21)。采集所有研究对象清晨空腹血清的 S100A12值及血红蛋白值,同时进行 Blatchford评分、 Rockall评分,根据 Blatchford评分将研究对象分为低危组和中高危组,用 ROC曲线分析血清 S100A12在中高危组急性非静脉曲张性上消化道出血严重程度的预测价值。结果轻、中、重度三组血清 S100A12水平分别为(34.85±8.28)、(62.78±17.65)、(153.05±22.42)ng/mL,各组 S100A12随着病情加重逐渐升高,差异有统计学意义(P<0.05)。中高危组 S100A12值[(92.87±47.00)ng/mL比(15.53±4.22)ng/mL]、 Rockall评分[(3.37±0.49)分比(1.93±0.55)分]明显高于低危组,差异有统计学意义(P<0.05)。中高危组血红蛋白低于低危组[(77.14±12.41)g/L比(132.63±7.57)g/L],差异有统计学意义(P<0.05)。 ③血清 S100A12与 Rockall评分、 Blatchford评分呈正相关,与血红蛋白呈负相关。 ROC曲线下面积:血清 S100A12(AUC=0.952)、 Rockall评分(AUC=0.935)、血红蛋白(AUC=0.823)。结论急性非静脉曲张性上消化道出血病人中 S100A12显著升高,与出血严重程度密切相关。

      Objective To study the correlation between serum S100 calcium binding protein 12(S100A12)and the severity of acute non?varicose upper gastrointestinal bleeding.Methods Ninety patients with acute non?varicose upper gastrointestinal bleed?ing who were diagnosed by emergency gastroscopy and admitted to the Affiliated Hospital of Weifang Medical University from Feb?ruary 2017 to August 2018 were selected.According to the severity of acute non?varicose upper gastrointestinal bleeding,the pa? tients were classified into mild level(n=27),moderate level(n=42)and severe level(n=21).The levels of S100A12 and hemo? globin content in the morning fasting serum of all subjects were collected,and evaluation of Blatchford score and Rockall score were conducted simultaneously.According to the Blatchford score,the subjects were divided into the low?risk group and the medium?high risk group.ROC curve was used to analyze the predictive value of serum S100A12 in the severity of acute non?varicose upper gastrointestinal bleeding in the middle and high?risk groups.Results The level of S100A12 in the mild,moderate and severe group was(34.85±8.28)(62.78±17.65),(153.05±22.42)ng/mL,respectively.S100A12 level increased with the aggravation of the dis?ease,andthedifferen,ce was statistically significant(P<0.05).The S100A12 level[(92.87±47.00)ng/mL vs.(15.53±4.22)ng/ mL]and Rockall score[(3.37±0.49)score vs.(1.93±0.55)score]in the severe group were significantly higher than those in the moderate group,the S100A12 level and Rockall score in the moderate group were significantly higher than those in the mild group(P<0.05).The hemoglobin level in the medium?high risk group was lower than that in the low?risk group[(77.14±12.41)g/L vs.(132.63±7.57)g/L],and the difference was statistically significant(P<0.05).Serum S100A12 was positively correlated with the Rockall score and Blatchford score,and negatively correlated with hemoglobin.Area under ROC curve:serum S100A12(AUC=0.952), Rockall score(AUC=0.935), hemoglobin(AUC=0.823).Conclusion S100A12 is significantly increased in patients with acute non?varicose upper gastrointestinal bleeding,which is closely related to the severity of bleeding.
