| 目的观察氟比洛芬酯复合右美托咪定对腹腔镜胆囊切除手术老年病人七氟醚阻滞肾上腺素能反应的最低肺泡有效浓度(MACBAR)的影响。方法选择 2018年 2月至 2019年 2月芜湖市第二人民医院行腹腔镜胆囊切除手术老年病人 44例，采用随机数字表法分为观察组 23例和对照组 21例。观察组在诱导前 15 min静脉注射右美托咪定 0.5 μg/kg(10 min注射完)，然后以 0.2 μg·kg-1·h-1维持，且插管完成后给予氟比洛芬酯 50 mg静脉注射。对照组给予等量生理盐水，术毕前给予氟比洛芬酯 50 mg静脉注射。麻醉诱导气管插管后机械通气，吸入七氟醚维持麻醉。观察气腹刺激后血流动力学变化，采用序贯法测定七氟醚 MACBAR。结果观察组平均动脉压(MAP)气腹后(79±7)mmHg较气腹前(75±9)mmHg比较，差异无统计学意义(P＞0.05)。对照组 MAP气腹后(82±7)mmHg较气腹前(73±7)mmHg明显升高，差异有统计学意义(P＜0.05)。观察组七氟醚 MACBAR为 1.85％(95%CI:1.45%~2.25%)对照组七氟醚 MACBAR为 2.35%(95%CI:2.19%~2.51%)，两组比较差异有统计学意义(P＜0.05)。观察组七氟醚 MACBAR较对照组，降低 21.3%。结论氟比洛芬酯复合右美托咪定可降低腹腔镜手术老年病人七氟醚 MACBAR。 |

| Objective To observe the effect of dexmedetomidine combined with flurbiprofen axetil on the minimum alveolar concen? tra?tion of sevoflurane(MACBRA)for blocking adrenergic response in elder patients during laparoscopic cholecystectomy surgery. Methods 44 elder patients with ASA Ⅰ?Ⅱ undergoing laparoscopic cholecystectomy surgery in Wuhu No.2 People’s Hospital from February 2018 to February 2019 were randomly divided into two groups，experiment group(group E，n＝23)and control group(group C，n＝21).Dexmedetomidine combined with flurbiprofen axetil were infused before pneumoperitoneum stimulation in groupE.The hemodynamic changes(MAP，HR)were observed after pneumoperitoneum stimulation.The MACBRA of sevoflurane in each group was measured by sequential method.Results The MACBAR Of sevoflurane was 1.85％(95％C1:1.45％?2.25％) in the group E and 2.35％(95％CI:2.19％?2.51％) in the group C(P＜0.05)，and 21.3％ less the group E was than the group C.Conclusion Dexmedetomidine combined with flurbiprofen axetil can reduce the MACBRA of sevoflurane in elder patients undergoing laparoscopic surgery by21.3%，and make hemodynamics become more smooth. |
