      目的探讨微小核糖核酸 ?155(miRNA?155)、骨桥蛋白(OPN)、嗜酸性粒细胞趋化因子(Eotaxin)水平与支气管哮喘病儿气管重塑的关系及其联合检测对病情诊断的价值。方法选择成都市温江区人民医院于 2016年 5月至 2018年 12月期间收治的 160例急性发作支气管哮喘病儿,根据高分辨率螺旋 CT诊断结果是否发生气管重塑分为重塑组 54例、非重塑组 106例,纳入该院同期住院的其他呼吸系统疾病病儿 60例为对照组。对入组病儿气管重塑指标、肺功能、 miRNA?155、OPN及 Eotaxin检测并对比。结果各组气管重塑指标和肺功能均有明显差异,其中对照组气管壁面积占气管总横断面积面积百分比(WA)(48.65±2.21)%,非重塑组 WA(70.62±3.79)%,重塑组 WA(86.44±5.02)%;重塑组 miRNA?155低于非重塑组,对照组最高,均差异有统计学意义(P<0.05)。各组 miRNA?155、OPN、Eotaxin指标均有明显差异, miRNA?155与 WA呈负相关性,与一秒率(FEV1/FVC)、一秒用力呼气容积(FEV1)呈正相关; OPN、Eotaxin与 WA呈正相关,与 FEV1/FVC、FEV1呈负相关。结论联合检测 miRNA?155、OPN、Eotaxin水平对支气管哮喘病儿气管重塑及病情严重程度评估具有一定的临床意义。

      Objective To investigate the relationship between the levels of microRNA?155(miRNA?155),osteopontin(OPN) and eotaxin levels and tracheal remodeling in children with bronchial asthma and the value of combined detection in the diagnosisof the disease.Methods A total of 160 cases of acute bronchial asthma children,who were hospitalized in People’s Hospital of Chengdu Wenjiang District from May 2016 to December 2018,were selected and assigned into remodeling group(n=54)and non? remodeling group(n=106)according to the results of high resolution spiral CT in the diagnosis of whether there was airway remod? eling.Meanwhile,60 cases of other respiratory inflammation admitted to hospital were included in control group.The tracheal remodel? ing indexes,lung function,miRNA?155,OPN and Eotaxin of the enrolled children were detected and compared.Results The tra? chea remodeling indexes and lung functions were significantly different among groups.The percentages of air wall area to total crosssectional area of trachea(WA)were(48.65±2.21)% in control group,(70.62±3.79)% in non?remodeling group and(86.44±5.02)% in remodeling group.MiRNA?155 in the remodeling group was lower than that in the non?remodeling group,with the highest in the control group,and there were statistically significant differences(P<0.05).MiRNA?155,OPN and Eotaxin indexes were significantlydifferent among groups.MiRNA?155 was negatively correlated with WA and positively correlated with FEV1/FVC(one second rate) and FEV1(forced expiratory volume per second).OPN and Eotaxin were positively correlated with WA,while negatively correlated with FEV1/FVC and FEV1.Conclusion The combined detection of miRNA?155,OPN,and Eotaxin levels has certain clinical signif?icance for airway remodeling in children with bronchial asthma and the evaluation of the severe condition of the disease.
