目的观察微小 RNA(miR)-497-3p在幽门螺杆菌(Hp)感染病儿胃黏膜组织中表达水平,并探讨其与核因子(NF)-κB信号通路的相关性。方法选取 2014年 1月至 2018年 11月黄冈市中心医院就诊的病儿 210例为研究对象,分为 Hp感染组(n = 100)和 Hp非感染组(n = 110)。实时荧光定量聚合酶链式反应(qRT-PCR)检测 miR-497-3p、Toll样受体 8(TLR8)、Toll样受体 10(TLR10)和 NF-κB mRNA的表达情况,酶联免疫吸附测定(ELISA)检测病儿胃黏膜组织中肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)和白细胞介素-10(IL-10)水平。结果 Hp感染组的胃黏膜组织中 miR-497-3p mRNA相对表达量显著低于 Hp非感染组[(1.53±0.16)比(2.46±0.14)],TLR8[(1.84±0.15)比(1.05±0.23)]、TLR10[(1.92±0.06)比(1.38±0.09)]和 NF-κB[(2.56±0.03)比(1.40±0.02)]相对表达量均高于 Hp非感染组,差异有统计学意义(均 P < 0.05);Hp感染组的胃黏膜组织中 TNF-α[(21.18±1.35)比(11.50±2.37)]、IL-6[(1.74±0.15)比(1.05±0.23)]、IL-8[(1.92±0.06)比(1.38±0.09)]和 IL-10[(2.56±0.03)比(1.40±0.02)]的表达水平均高于 Hp非感染组,均差异有统计学意义;Pearson相关分析显示,Hp感染组病儿胃黏膜组织中 miR-497-3p与 TLR8、TLR10和 NF-κB mRNA相对表达量、TNF-α、IL-6、IL-8、IL-10水平均呈负相关(均 P < 0.05)。 miRNA-497-3p诊断 Hp感染的 ROC曲线下面积(AUC)为 0.926,根据 SPSS统计结果计算 miR-497-3p在最佳临界值处诊断 Hp敏感度和特异度分别为 91.8%和 93.7%。结论 miR-497-3p在 Hp感染组的胃黏膜组织中异常低表达,其可能通过影响 TLR8/TLR10/NF-κB信号通路介导的炎性反应,从而在 Hp感染的致病过程中发挥作用,为临床下一步的治疗提供思路。 |

Objective To investigate the expression level of microRNA (miR)-497-3p in gastric mucosa of children infected with Helicobacter pylori (Hp),and to explore its correlation with nuclear factor (NF)-κB signaling pathway.Methods A total of 210 cases treated in Huanggang Central Hospital from January 2014 to November 2018 were selected and assigned into Hp infection group (n = 100)and Hp non-infection group (n = 110). Real-time fluorescent quantitative PCR (qRT-PCR) was used to detect the expressions of miR497-3p, TLR8, TLR10 and NF-κB mRNA, and enzyme-linked immunosorbent assay (ELISA) was used to detect the levels of tumor necrosis factor (TNF-α) , interleukin-6 (IL-6), IL-8 and IL-10 in gastric mucosa tissue of the children.Results In Hp infection group, the relative expression of miR-497-3p in gastric mucosa tissue was significantly lower than that in Hp non-infection group [(1.53±0.16) vs.(2.46±0.14)],and the relative expressions of TLR8 [(1.84±0.15)vs. (1.05±0.23)],TLR10 [(1.92±0.06) vs.(1.38±0.09)]and NF-κB [(2.56±0.03) vs. (1.40±0.02)] were higher than those in the Hp non-infection group; the differences were statistically significant (all P < 0.05). The expression levels of TNF-α [(21.18±1.35)vs.(11.50±2.37)],IL-6 [(1.74±0.15) vs. (1.05±0.23)], IL-8 [(1.92±0.06) vs. (1.38±0.09)] and IL-10 [(2.56±0.03) vs. (1.40±0.02)] in gastric mucosa tissue were higher in HP infection group than in Hp non-infection group, with statistically significant differences. The Pearson correlation analysis showed that the expression of miR-497-3p in gastric mucosa tissue in HP infection group was negatively correlated with the relative expression levels of TLR8, TLR10, NF-κB mRNA, and the relative expression levels of TNF-α,IL-6,IL-8 and IL-10(all P < 0.05).Area under the ROC curve (AUC) of miRNA-497-3p in the diagnosis of Hp infection was 0.926. The sensitivity and specificity of miR-497-3p at the optimal critical value were 91.8% and 93.7% on the basis of the statistical results of SPSS.Conclusion The abnormally low level of miR-497-3p in gastric mucosa tissue of children in HP infection group may play a role in the process of HP infection by inflammatory response affecting the TLR8/TLR10/NF-κB signaling pathway mediation,which provides clinical ideas for further treatment. |
