      目的探讨糖尿病酮症酸中毒与糖化血清白蛋白的相关性。方法 2018年 6月至 2019年 7月在武汉大学人民医院收治的 114例 2型糖尿病病人,按其是否并发酮症酸中毒分为酮症酸中毒组( DKA组, 47例)和糖尿病组(非 DKA组, 67例),并收取同期来该院体检的健康人群 48例为健康对照组。比较分析三组间糖化血清白蛋白( GA)、糖化血红蛋白( HbA1c)、空腹血糖、谷丙转氨酶( ALT)、谷草转氨酶( AST)、碱性磷酸酶( ALP)、总蛋白、白蛋白、尿素、肌酐、尿酸、预估肾小球滤过率( eGFR)、总胆固醇、三酰甘油、低密度脂蛋白胆固醇( LDL-C)、高密度脂蛋白胆固醇( HDL-C)水平的差异以及 GA与上述指标的相关性。结果 DKA组 GA水平( 12.72±5.03)g/L显著高于非 DKA组( 9.25±3.09)g/L和对照组( 5.25±1.11)g/L,三组间差异有统计学意义(P<0.01);且 DKA组 HbA1c、空腹血糖、 ALP、尿素、尿酸水平显著高于非 DKA组和对照组,三组间差异有统计学意义(P<0.01); DKA组总蛋白、白蛋白、 HDL-C水平显著低于非 DKA组和对照组,三组间差异有统计学意义( P<0.01)。相关性分析显示, GA与 HbA1c、空腹血糖、 ALP、尿素呈显著的正相关( P<0.01),与总蛋白、白蛋白、 HDL-C呈显著的负相关( P<0.01)。结论 DKA病人血清 GA水平显著增高,监测血清 GA水平可用于预测 DKA的发生。 DKA病人总蛋白、白蛋白水平增高, ALP水平降低,提示其可能伴有肝功能受损;尿素、尿酸水平升高,提示可能伴有肾功能受损。

      Objective To investigate the correlation between diabetic ketoacidosis and glycosylated serum albumin. Methods From June 2018 to July 2019,the 114 patients with type 2 diabetes admitted to the people′s hospital of wuhan university were dividedinto the ketoacidosis group (DKA group, 47 cases) and the diabetes group (non-dka group, 67 cases) according to whether they werecomplicated with ketoacidosis or not, and 48 healthy patients who received physical examinations from our hospital during the same pe?riod were enrolled as healthy control group. We compared and analyzed the difference of glycosylated serum albumin (GA), glycosylat?ed hemoglobin (HbA1c), fasting plasma glucose (FPG), alanine transaminase (ALT), aspartate transaminase (AST), alkaline phospha?tase (ALP), total protein (TP), albumin (ALB), urea, serum creatinine (Scr), uric acid (UA), estimated glomerular filtration rate (eGFR),total cholesterol (TC), triglyceride (TG), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol(HDL-C)level among the three groups and the correlation between GA and the above indexes.Results The levels of GA (12.72±5.03)g/L in the DKA group were significantly higher than those in the non-DKA group (9.25±3.09)g/L and the control group (5.25±1.11)g/L (P<0.01). Thelevels of HbA1c, FPG, ALP, urea, creatinine and uric acid in the DKA group were also significantly higher than those in the non-DKA group and the control group (P<0.01).The TP, ALB and HDL-c levels in the DKA group were significantly lower than those in the non-DKA group and the control group (P<0.01).Correlation analysis showed that GA was significantly positively correlated with HbA1c, FPG, ALP and urea (P<0.01) and negatively correlated with TP, ALB and HDL-c(P<0.01). Conclusion The level of serum GA in pa?tients with DKA is significantly increased.The level of serum GA can be used to predict the occurrence of DKA. In patients with DKA,the levels of TP and ALB increased and the levels of ALP decreased, suggestes that the levels of urea and UA may be accompanied bydamage of liver function, and the levels of urea, and UA may be accompanied by renal function damage.
