      目的探讨 α-平滑肌肌动蛋白( α-SMA)、上皮钙黏素( E-cadherin)在腹膜透析大鼠腹膜组织中的表达及意义。方法 SPF级 SD大鼠 45只,抽签选取 26只采用 5/6肾切除法制备大鼠尿毒症模型,将造模成功的 21只再抽签选取 20只分为模型组和腹膜透析组各 10只;另剩余 19只大鼠抽签选取 10只作为假手术组。术后 5周建立腹膜透析模型;腹膜透析组每天灌注 4.25%腹膜透析液 20 mL,持续 6周。停止透析 24 h后测定腹膜超滤量、透出液肌酐( D-Scr)、血肌酐( P-Scr)、初始腹透液糖(D0)及透出液糖( D1)浓度;苏木精 -伊红( HE)和 Masson染色观察腹膜组织病理变化;反转录聚合酶链反应( RT-PCR)和免疫组化测定腹膜组织 α-SMA、E-cadherin蛋白和 mRNA的表达; Pearson相关性分析 α-SMA和 E-cadherin表达量之间的相关性, αSMA、E-cadherin与腹膜结构和功能之间的相关性。结果与模型组相比,腹膜透析组大鼠的腹膜超滤量和 D1/D0明显下降(P <0.05)D/P-Scr、腹膜厚度明显增加( P<0.05);与假手术组相比,模型组和腹膜透析组大鼠 α-SMA的 mRNA水平明显上调[(0.56±0.,05),(0.92±0.09)(1.38±0.12)P<0.05],E-cadherin的 mRNA水平明显下调[(0.41±0.04)(0.35±0.04)(0.24±0.03),P <0.05]且相比模型组,腹膜透,析组大鼠α-,SMA的 mRNA水平明显上调, E-cadherin的mRNA水平明显,下调( P<0.0,5)各组大鼠 α-SMA和,E-cadherin的 mRNA水平呈负相关关系( P<0.05);与假手术组相比,模型组和腹膜透析组大鼠 α-SMA的蛋白,表达明显上调[( 0.42±0.05),(0.83±0.08)(1.25±0.11)P<0.05]E-cadherin的蛋白表达明显下调[( 0.32±0.04)(0.28±0.03)(0.21±0.03)P<0.05]且相比模型组,-SMA白表达明显上调, 调( P<0.05,各组MA和 Ecadherin的蛋白表达呈负相关关系( P<0.05);大鼠腹膜组织中 α-SMA的 mRNA水平与腹膜厚度和 D/P-Scr呈正相关关系( P<0.05),与腹膜超滤量和 D1/D0呈负相关关系( P<0.05)E-cadherin与腹膜厚度和 D/P-Scr呈负相关关系( P<腹膜透,析组大鼠α,的蛋,E-cadherin的蛋白表达明显下,),大鼠α-S,-,0.05),与腹膜超滤量和 D1/D0呈正相关关系( P<0.05)。结论腹膜透鼠腹膜组织中 α-SMA表达明显上调, E-cadherin表达明显下调, α-SMA和 E-cadherin的表达与腹膜的结构和功能具有相关性。

      Objective To explore expression and significance of α-smooth muscle actin (α-SMA) and E-cadherin in peritoneal tissue of rats undergoing peritoneal dialysis (PD). Methods Of 45 SPF SD rats,26 rats were selected by lottery, and 21 of them were success?fully obtained by modeling. Twenty of them were divided into model group and peritoneal dialysis group with 10 each. The remaining19 rats were selected by lottery as sham operation group. In model group and PD group, rat uremia models were established by 5/6 ne?phrectomy. At 5 weeks after surgery, PD models were established. PD group was perfused with 4.25% peritoneal dialysate of 20 mL perday for 6 weeks. At 24h after stopping dialysis, Peritoneal ultrafiltration volume, concentration of dialysate creatinine (D-Scr), plasma creatinine (P-Scr), initial peritoneal dialysate glucose (D0) and dialysate glucose (D1) were measured. HE and Masson staining were ap?plied to observe pathological changes of peritoneal tissues. The expressions of α-SMA, E-cadherin protein and mRNA in peritoneal tis? sues was detected by RT-PCR and immunohistochemistry. The correlation between α-SMA and E-cadherin expression, correlation be? tween α-SMA, E-cadherin and peritoneal structure and function were analyzed by Pearson correlation analysis. Results Compared with model group, peritoneal ultrafiltration volume and D1/D0 were significantly decreased in PD group (P<0.05), while D/P-Scr and peritoneal thickness were significantly increased (P<0.05). Compared with the sham operation group, the mRNA level of α -SMA in the model group and peritoneal dialysis group was significantly increased [(0.56 ± 0.05), (0.92 ± 0.09), (1.38 ± 0.12), P<0.05], and the mRNA level of E-cadherin was significantly decreased [(0.41 ± 0.04), (0.35 ± 0.04), (0.24 ± 0.03), P<0.05],The mRNA levels of α SMA and E-cadherin were negatively correlated in each group (P<0.05); compared with the sham operation group, the protein expres? sion of α -SMA in the model group and peritoneal dialysis group was significantly increased [(0.42 ± 0.05), (0.83 ± 0.08), (1.25 ± 0.11), P<0.05], and the protein expression of E-cadherin was significantly down-regulated [(0.32 ± 0.04), (0.28 ± 0.03), (0.21 ± 0.03), P<0.05], In peritoneal dialysis group, the protein expression of α -SMA was significantly up-regulated, and the protein expression of E-cadherin was significantly down-regulated (P<0.05). There was a negative correlation between the protein expression of α -SMA and E-cadherin (P<0.05);The α-SMA was significantly positively correlated with peritoneal thickness and D/P-Scr (P<0.05), while significantly nega? tively correlated with peritoneal ultrafiltration volume and D1/D0 (P<0.05). E-cadherin was significantly negatively correlated with peri? toneal thickness and D/P-Scr (P<0.05), while significantly positively correlated with peritoneal ultrafiltration volume and D1/D0 (P< 0.05). Conclusion The expression of α-SMA was significantly up-regulated, while expression of E-cadherin was significantly down-regulated in peritoneal tissues of rats undergoing PD. The expression of α-SMA and E-cadherin was significantly correlated with struc? ture and function of peritoneum.
