      目的探讨成都医学院第一附属医院 10年间呼吸科铜绿假单胞菌的耐药性和流行病学变化特征。方法收集 2007— 2008年和 2017—2018年间成都医学院第一附属医院呼吸科住院病人临床标本分离出的铜绿假单胞菌,采用琼脂平板倍比稀释法测定抗菌药物对铜绿假单胞菌的最低抑菌浓度,多位点序列分型分析菌株的基因型差异, PCR扩增测定菌株的碳青霉烯酶基因。结果 2007—2008年和 2017—2018年间从呼吸科住院病人分别收集 134株和 85株铜绿假单胞菌,其检出率为 4.3%和 2.0%。2007—2008年间铜绿假单胞菌对抗菌药物普遍耐药,高于 2017—2018年间菌株的耐药率(P<0.01)。 MLST结果显示 2007—2008年间菌株具有 15个基因型, 2017—2018年间菌株具有 9个基因型,其中 ST235、ST111和 ST175为最流行的基因型,菌株的基因型 10年间没有明显变化。 2007—2008年间共有 14株耐碳青霉烯铜绿假单胞菌携带金属 β-内酰胺酶基因 IMP-4,5株携带金属 β-内酰胺酶基因 VIM-2,而 2017—2018年间仅 2株菌携带 IMP-4基因,无其他金属酶基因存在。结论该院呼吸科铜绿假单胞菌耐药性具有明显下降的趋势, MLST基因型谱稳定遗传,且 IMP和 VIM仍是铜绿假单胞菌的一个重要耐药机制。

      Objective To investigate the resistance and epidemiological characteristics of Pseudomonas aeruginosa isolated from therespiratory department in The First Affiliated Hospital of Chengdu Medical College during the past 10 years.Methods The P. aerugi?nosa isolates were collected from the clinical specimens of inpatients in the respiratory department of the First Affiliated Hospital ofChengdu Medical College in 2007-2008 and 2017-2018. The minimum inhibitory concentration of antibacterial drugs against P. aerugi?nosa was determined by agar plate dilution method. The difference of bacterial genotypes was analyzed by multilocus sequence typing(MLST), and the carbapenemase genes were determined by PCR amplification.Results A total of 134 and 85 P. aeruginosa isolates were collected from inpatients in 2007-2008 and 2017-2018, and the detection rates were 4.3% and 2.0%, respectively. In 2007-2008,P. aeruginosa strains had a high resistance to almost all of the antibiotics tested, which was higher than that of the strains in 2017-2018 (P<0.01). The MLST results showed 15 genotypes in 2007-2008 and 9 genotypes in 2017-2018. ST235, ST111 and ST175 were themost prevalent genotypes, and there was no significant change in the genotypes during the past 10 years. In 2007-2008, 14 carbapenemresistant P. aeruginosa carried the IMP-4 gene, and 5 strains carried the VIM-2 gene. In 2017-2018, only 2 strains carried the IMP-4 gene, and no other genes were found.Conclusion The results indicate that the resistance of P. aeruginosa in our hospital is significant?ly decreased. There is a stable MLST genotype spectrum, and IMP and VIM are still an important resistance mechanism of P. aerugino?sa.
