      目的探究微小 RNA361-5p(miR-361-5p)在肺结核病人血清中水平及意义。方法选取 2017年 3月至 2019年 1月开封市结核病防治所诊治的肺结核病人 136例为肺结核组;并选择同时间段内同一医院 142例健康体检者为健康对照组。比较两组一般临床资料;以实时荧光定量 PCR(qRT-PCR)法检测两组血清 miR-361-5p水平;检测血管内皮生长因子( VEGF)、肿瘤坏死因子 -α(TNF-α)、 γ-干扰素(IFN-γ)水平;分析肺结核病人血清 miR-361-5p水平与 VEGF、TNF-α、IFN-γ的关系;回归分析肺结核发生的影响因素;分析血清 miR-361-5p对肺结核的诊断价值。结果肺结核组血清 miR-361-5p[( 1.58±0.45)比( 1.06±0.31)]、 VEGF[(389.39±46.85)pg/mL比( 327.62±41.76)pg/mL]、 TNF-α[( 22.48±5.06)ng/L比( 11.36±3.17)ng/L]水平均明显高于健康对照组, IFN-γ水平[( 15.58±3.36)ng/L比( 26.72±4.78)ng/L]明显低于健康对照组(均 P <0.05);肺结核病人血清 miR-3615p与 VEGF、TNF-α水平呈正相关,与 IFN-γ水平呈负相关(均 P <0.05); miR-361-5p、VEGF、TNF-α是影响肺结核发生的危险因素, IFN-γ是影响肺结核发生的保护因素(均 P <0.05);血清 miR-361-5p水平对肺结核发生诊断的 AUC为 0.898,截断值为 1.44,其灵敏度为 83.8%,特异度为 88.0%。结论肺结核病人血清 miR-361-5p呈高表达,与炎症相关因子密切相关, miR-361-5p可能与炎症相关因子相互作用,进而共同调节肺结核发生、发展。

      Objective To explore the level and significance of microRNA361-5p (miR-361-5p) in serum of patients with pulmonary tuberculosis.Methods 136 cases of pulmonary tuberculosis patients treated in Kaifeng Tuberculosis Prevention and Treatment Centerfrom March 2017 to January 2019 were analyzed and studied, and they were called pulmonary tuberculosis group; in the same period,142 healthy persons in our hospital were selected as the control group. The general clinical data of the two groups were compared andanalyzed; the level of serum miR-361-5p was detected by real-time fluorescence quantitative PCR (qRT-PCR); the levels of vascular endothelial growth factor (VEGF), tumor necrosis factor-α (TNF-α) and interferon-γ (IFN-γ) were measured; the relationships between serum level of miR-361-5p and levels of VEGF, TNF-α and IFN-γ in patients with pulmonary tuberculosis were analyzed; the influencing factors of pulmonary tuberculosis were analyzed by regression analysis; and the diagnostic value of serum miR-361-5p in pulmonary tuberculosis was analyzed. Results The levels of serum miR-361-5p[(1.58±0.45) vs. (1.06±0.31)], VEGF[(389.39±46.85)pg/mL vs. (327.62±41.76)pg/mL] and TNF-α[(22.48±5.06)ng/L vs.(11.36±3.17)ng/L] in tuberculosis group were significantly higher than those in healthy group, and the level of IFN-γ[(15.58±3.36)ng/L vs.(26.72±4.78)ng/L] in tuberculosis group was significantly lower than that in healthy group (P<0.05); the serum level of miR-361-5p was positively correlated with levels of VEGF and TNF-α, and negatively correlated with level of IFN-γ in patients with pulmonary tuberculosis (P<0.05); miR-361-5p, VEGF and TNF-α were risk factors for tuberculosis, and IFN-γ was protective factor for tuberculosis (all P<0.05); the AUC of serum miR-361-5p level for diagnosis of pulmonarytuberculosis was 0.898, the truncation value was 1.44, the sensitivity was 83.8%, and the specificity was 88.0%.Conclusion The expression of serum miR-361-5p in patients with pulmonary tuberculosis is high, which is closely related to inflammatory factors, miR361-5p may interact with inflammatory factors, and then co-regulate the occurrence and development of pulmonary tuberculosis.
