      目的分析程序性死亡因子 -1(programmed death factor-1,PD-1)、程序性死亡配体 -1(programmed death-ligand-1,PDL1)、 TNF家族 B细胞活化因子 -R(B cell-activating factor-R belonging to the TNF family,BAFF-R)在弥漫大 B细胞淋巴瘤组织中的差异性表达及临床意义。方法以延安大学附属医院血液科 2019年 1月至 2020年 4月初诊的 28例弥漫性大 B细胞淋巴瘤组织及瘤旁组织样本作为研究对象,采用免疫组化法检测其 PD-1、PD-L1及 BAFF-R表达,然后统计分析 PD-1、PD-L1及 BAFF-R表达与病人临床病理特征的关系。结果弥漫性大 B细胞淋巴瘤组织 PD-1、PD-L1及 BAFF-R阳性表达积分分别为(5.44±0.79)、(5.58±0.86)及( 4.49±0.65)分,瘤旁组织 PD-1、PD-L1及 BAFF-R阳性表达积分分别为( 3.31±0.35)、(3.25±0.34)及( 2.77±0.27)分,差异有统计意义( P<0.05);淋巴瘤组织 PD-1表达差异与临床病理特征差异无统计学意义( P>0.05);淋巴瘤组织 PD-L1表达与 Ann Arbor/Costwards分期、病理分型及 EBV是否阳性差异有统计意义( P<0.05);淋巴瘤组织 BAFF-R表达与 Ann Arbor/Costwards分期、病理分型、 IPI评分及 EBV是否阳性差异有统计意义( P<0.05)。结论 PD-L1及 BAFF-R在弥漫性大 B细胞淋巴瘤组织中呈高表达,且与弥漫性大 B细胞淋巴瘤分期、病理分型及 EBV感染密切相关,二者联合检测对病人预后评估可能具有较高价值;对难治性弥漫大 B细胞淋巴瘤而言,以 PD-L1及 BAFF-R的信号通路为靶点,是潜在的免疫治疗方案。

      Objective Analysis of the differential expression and clinical significance of programmed death factor-1 (PD-1), pro grammed death-ligand-1 (PD-L1) and B-cell activating factor receptor (BAFF-R) in diffuse large B-cell lymphoma tissues.Methods Twenty-eight cases of diffuse large B-cell lymphoma tissue and adjacent tissue samples from the Department of Hematology, Yan'anUniversity Affiliated Hospital from January 2019 to April 2020 were selected as the research objects, and their PD-1, PD-L1 and BAFFR expression was detected by immunohistochemistry methods. Then, the relationship between PD-1, PD-L1 and BAFF-R expression and the clinicopathological characteristics of patients was statistically analyzed.Results The positive expression scores of PD-1, PDL1 and BAFF-R in diffuse large B-cell lymphoma tissue were 5.44±0.79, 5.58±0.86 and 4.49±0.65 points, respectively. The positive ex pression scores of PD-1, PD-L1 and BAFF-R in adjacent tissue were 3.31±0.35, 3.25±0.34 and 2.77±0.27 points, respectively, and the difference was statistically significant (P < 0.05). There was no significant difference between the expression of PD-1 and clinicopatho logical characteristics in lymphoma tissue (P > 0.05). The expression of PD-L1 in lymphoma tissue was significantly different from Ann Arbor/Costwards staging, pathological type, and EBV positivity (P < 0.05). The expression of BAFF-R in lymphoma tissue was signifi cantly different from Ann Arbor/Costwards staging, pathological type, IPI score and whether EBV was positive (P < 0.05).Conclu sions PD-L1 and BAFF-R are highly expressed in diffuse large B-cell lymphoma tissue and are closely related to the stage, pathologi cal type and EBV infection of diffuse large B-cell lymphoma. The combined detection of PD-L1 and BAFF-R may be useful in evaluat ing the prognosis of patients. For refractory diffuse large B-cell lymphoma, targeting the signaling pathways of PD-L1 and BAFF-R isa potential immunotherapy regimen that has high value.
