      目的探讨核苷酸结合寡聚化结构域样受体蛋白 3(NLRP3)、胱天蛋白酶 -1(caspase-1)、白细胞介素 -1β(IL-1β)在子宫内膜腺癌组织中的表达及临床意义。方法选择徐州市中心医院 2014年 1月至 2016年 12月行手术治疗的病人组织蜡块标本,其中 121例子宫内膜腺癌、 32例子宫内膜不典型增生及 50例正常增殖期子宫内膜,免疫组化法检测上述组织中 NLRP3、 caspase-1、IL-1β的阳性表达,分析 NLRP3、caspase-1、IL-1β表达水平与子宫内膜腺癌病人 5年生存率的关系。结果子宫内膜腺癌组病人中 NLRP3、caspase-1的阳性率为 62.0%、71.1%,在子宫内膜不典型增生组织中的阳性率为 46.9%、62.5%,其表达水平明显高于正常增殖期子宫内膜组织中 NLRP3、caspase-1的阳性率 14%,22%(P<0.05)。子宫内膜腺癌病人组织中 NLRP3表达与 caspase-1、IL-1β表达呈正相关( r=0.40,P<0.001;r=0.22,P=0.015),caspase-1与 IL-1β表达呈正相关( r=0.27,P=0.003)。 Kaplan-Meier生存分析显示: NLRP3、caspase-1、IL-1β阳性表达病人 5年生存率分别为 77.3%(58/75)、 80.2%(69/86)、 79.1%(53/ 67)均显著短于阴性表达病人 97.8%(45/46)、 97.1%(34/35)、 92.6%(50/54)(均 P<0.05)。 Cox回归分析显示 NLRP3是子宫内膜腺癌,病人预后的独立危险因素[ HR=15.375,95%CI:(1.595,148.187)P=0.018]。结论 NLRP3、caspase-1、IL-1β可能通过细胞焦亡参与子宫内膜癌的发生发展; NLRP3高表达是影响子宫内膜腺癌,病人预后的独立危险因素,可作为潜在的预后分子标志物,为肿瘤提供有效的治疗靶点。

      Objective To investigate the expression and clinical significance of nucleotide-binding oligomerization domain-like re. ceptor protein 3 (NLRP3), caspase-1 and interleukin-1β (IL-1β) in endometrial adenocarcinoma tissues.Methods The paraffin tissuespecimens of patients who underwent surgical treatment from January 2014 to December 2016 in Xuzhou Central Hospital were select.ed, including 121 cases of endometrial adenocarcinoma, 32 cases of endometrial atypical hyperplasia and 50 cases of normal proliferat.ing endometrium. Immunohistochemistry was used to detect the expression of NLRP3, caspase-1 and IL-1β in the above tissues, and the relationship between the expression levels of NLRP3, caspase-1 and IL-1β and the 5-year survival rate of patients with endometrial adenocarcinoma was analyzed.Results The positive rates of NLRP3 and caspase-1 were 62.0% and 71.1% in the endometrial adeno. carcinoma group and 46.9% and 62.5% in tissues with atypical endometrial hyperplasia, respectively, and their expression levels weresignificantly higher than those of normal proliferating endometrial tissues by 14% and 22%, respectively (P<0.05). In tissues of endome. trial adenocarcinoma patients, the expression level of NLRP3 was positively correlated with caspase-1 and IL-1β expression (r=0.40, P< 0.001; r=0.27. P=0.003), and caspase-1 expression was positively correlated with IL-1β expression (r=0.27, P=0.003). Kaplan.Meier survival analysis showed that the 5-year survival rates of patients with positive expression of NLRP3, caspase-1, and IL-1β were 77.3% (58/75), 80.2% (69/86) and 79.1% (53/67), respectively, all of which were significantly shorter than those of patients with negative ex.pression [97.8% (45/46), 97.1% (34/35) and 92.6% (50/54)] (P<0.05). Cox regression analysis showed that NLRP3 was an independentrisk factor for prognosis in patients with endometrial adenocarcinoma [HR=15.375, 95%CI:(1.595, 148.187), P=0.018].Conclusion NLRP3, caspase-1 and IL-1β may be involved in the development of endometrial cancer through pyroptosis; high NLRP3 upregulationis an independent risk factor affecting the prognosis of patients with endometrial adenocarcinoma and may serve as a potential prognos.tic molecular marker to provide an effective therapeutic target for tumors.
