      目的探讨剪切波弹性成像( SWE)对甲状腺髓样癌( MTC)和甲状腺乳头状癌( PTC)的鉴别诊断价值。方法回顾性分析 2005年 2月至 2021年 5月四川大学华西医院经病理诊断的 32例 MTC和 127例 PTC病人 SWE资料,采用 Mann-Whitney U检验对两者 SWE参数进行对比分析,对其中差异有统计学意义的参数进行受试者操作特征曲线( ROC)分析。结果 MTC杨氏模量平均值( Emean)为 23.50(12.40)kPa小于 PTC Emean 36.15(18.65)kPa,差异有统计学意义,(P<0.001); MTC杨氏模量最小值( Emin)为 1.50(7.80)kPa小于 PTC Emin 13.70(20.25)kPa,差异有统计学意义( P<0.001); MTC与 PTC杨氏模量最大值(Emax)、结节与周围正常组织的杨氏模量比值( Eratio)比较,均差异无统计学意义( P>0.05)。 ROC曲线分析显示 Emean截断值为 27.80 kPa,ROC曲线下面积( AUC)为 0.77,灵敏度为 0.72,特异度为 0.76。Emin截断值为 10.10 kPa,AUC为 0.75,灵敏度为 0.60,特异度为 0.89。结论 SWE能为 MTC、PTC的鉴别诊断提供更多的依据,可作为常规超声的有效补充。

      Objective To evaluate the value of shear-wave elastography (SWE) in the differential diagnosis of medullary thyroid car-cinoma (MTC) and papillary thyroid carcinoma.Methods SWE data of 32 patients with MTC and 127 patients with PTC diagnosed bypathology in West China Hospital of Sichuan University from February 2005 to May 2021 were retrospectively analyzed. The SWE pa-rameters of the two were compared by the Mann-Whitney U test. The statistically significant parameters were analyzed by receiver oper-ating characteristic curve (ROC).Results The mean Young's modulus (Emean) of MTC was 23.50 (12.40) kPa and 36.15 (18.65) kPaof PTC Emean, the difference was statistically significant (P<0.001). The minimum Young's modulus (Emin) of MTC was 1.50 (7.80)kPa, which was lower than PTC Emin 13.70 (20.25) kPa, and the difference was statistically significant (P<0.001). The maximumYoung's modulus (Emax) between MTC and PTC, and the ratio of Young's modulus (Eratio) between nodules and surrounding normaltissues were not significantly different (P>0.05). ROC curve analysis showed that the cut-off value of Emean was 27.80 kPa, the area un-der the ROC curve (AUC) was 0.77, sensitivity was 0.72, and specificity was 0.76. The Emin cut-off value was 10.10 kPa, The AUC was 0.75, the sensitivity was 0.60, and the specificity was 0.89.Conclusion SWE can provide more basis for differential diagnosis of MTCand PTC and can be used as an effective supplement to conventional ultrasound.
