      目的 探讨无毛蛋白(hairless protein,Hr)在乳腺癌变和进展过程中的表达及临床意义。方法 收集2007年1月至2015年12月石河子大学医学院第一附属医院石蜡包埋乳腺癌旁正常组织81例(距癌组织>5 cm)、普通型导管增生79例、不典型导管增生26例、导管原位癌64例、浸润性导管癌138例。免疫组织化学法和数据库检测Hr在乳腺癌旁正常组织、普通型导管增生、不典型导管增生、导管原位癌、浸润性乳腺癌中表达,分析Hr表达与病人临床病理参数间关系及对预后的影响。结果 浸润性导管癌中Hr蛋白免疫组织化学评分[(4.14±3.78)分]显著低于乳腺癌旁正常组织[(6.28±4.43)分]、普通型导管增生[(6.85±4.20)分]、不典型导管增生[(6.12±3.84)分]、导管原位癌[(5.41±3.86)分],五组间差异有统计学意义(P<0.001)。Hr表达与淋巴结转移、TNM晚分期和雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体-2(HER-2)阴性状态(P<0.05)显著相关,Hr蛋白在骨转移乳腺癌中高表达(P=0.001)且与骨转移显著正相关(P=0.005)。Hr高表达的乳腺癌病人无复发生存率和总生存率显著降低(P<0.05)。结论 Hr在乳腺癌变和早期浸润阶段发挥抑癌基因功能,在肿瘤恶性进展和转移尤其骨转移过程中起促进作用,与病人预后不良密切相关。

      Objective To investigate the expression and clinicopathological significance of Hairless (Hr) protein in the transforma-tion and progression of breast cancer.Methods From January 2007 to December 2015, 81 cases of paraffin-embedded normal tissue adjacent to breast cancer (> 5 cm away from cancer tissue), 79 cases of usual ductal hyperplasia, 26 cases of atypical ductal hyperpla-sia, 64 cases of ductal carcinoma in situ, and 138 cases of invasive ductal carcinoma were collected from the First Affiliated Hospital of Shihezi University School of Medicine. Immunohistochemical and databases were applied to detect Hr expression in adjacent normal breast tissues, usual ductal hyperplasia, atypical ductal hyperplasia,ductal carcinoma in situ, and invasive ductal carcinoma to analyze the relationship between Hr expression and clinicopathological parameters of patients and its effect on prognosis.Results The immu-nohistochemical score of Hr protein in invasive ductal carcinoma (4.14±3.78) was significantly lower than that in normal tissues adja-cent to breast cancer (6.28±4.43), usual ductal hyperplasia (6.85±4.20), atypical ductal hyperplasia (6.12±3.84) and ductal carcinoma in situ (5.41±3.86), and the differences among the five groups were statistically significant (P<0.001). Hr expression was significantly correlated with lymph node metastasis, TNM late stage and negative status of estrogen receptor (ER), progesterone receptor (PR) and hu-man epidermal growth factor receptor 2 (HER-2) (P<0.05). Hr protein is highly expressed in breast cancer with bone metastasis (P=0.001) and significantly positively correlated with bone metastasis (P=0.005). The relapse-free and overall survival rates of breast can-cer patients with high Hr expression were significantly reduced (P<0.05).Conclusion Hr plays a role of tumor suppressor gene in the transformation and early invasion stage of breast cancer, and plays a promoting role in malignant progression and metastasis of tumors, especially in the process of bone metastasis, which is closely related to the poor prognosis of patients.
