目的 检测维吾尔族成人急性髓系白血病(非急性早幼粒细胞白血病)[AML(非M3)]病人骨髓肾母细胞瘤基因1(WT1)和转录因子ETS-1表达水平,并探讨二者与AML(非M3)预后的关系。方法 选取2015年6月至2018年10月喀什地区第一人民医院血液科收住的105例维吾尔族初诊AML(非M3)病人为观察组。选取同期于该院100例健康体检者为对照组A,及在该院治疗的100例汉族初诊AML(非M3)病人为对照组B。采用实时荧光定量逆转录聚合酶链反应(qRT-PCR)检测病人骨髓WT1基因和转录因子ETS-1的表达水平;多因素logistic回归分析AML的影响因素;采用Kaplan-Meier法描述生存曲线。结果 观察组和对照组B白细胞计数明显高于对照组A(P<0.05),血小板计数及血红蛋白明显低于对照组A(P<0.05);观察组和对照组B病人白细胞计数、血小板计数、血红蛋白比较,差异无统计学意义(P>0.05)。观察组骨髓WT1、ETS-1表达水平分别为0.83±0.15、0.92(0.22,1.23),明显高于对照组B的0.35±0.14、0.48(0.24,1.01)和对照组A的0.01±0.01、0.02(0.01,0.03)(P<0.05),对照组B骨髓WT1、ETS-1表达水平明显高于对照组A(P<0.05)。logistic回归分析显示,WT1、ETS-1均是AML的危险因素;以AML(非M3)病人WT1、ETS-1中位数为界分为低表达组与高表达组,K-M显示WT1、ETS-1低表达组生存率高于高表达组(P<0.05)。结论 维吾尔族成人AML(非M3)病人骨髓WT1和ETS-1水平呈高表达,且WT1和ETS-1高表达与疾病的发生及病人预后不良有关。 |

Objective To detect the expression levels of Wilms′ tumor 1 gene (WT1) and transcription factor ETS-1 in patients with acute myeloid leukemia (non acute promyelocytic leukemia) [AML (non-M3)] in Uygur adults, and to explore the relationship between WT1 and ETS-1 and the prognosis of AML (non-M3).Methods One hundred and five Uygur newly diagnosed AML (non-M3) patients admitted to the Hematology Department of The First People's Hospital of Kashi Region from June 2015 to October 2018 were selected as the observation group, in the same period, 100 healthy subjects in our hospital were selected as control group A, and 100 cases of AML (non-M3) who were treated in our hospital as control group B. The expression levels of WT1 gene and ETS-1 in bone marrow of pa-tients were detected by quantitative Real-time PCR (qRT-PCR), Multi-factor logistic regression analysis was used to analyze the influ-encing factors of AML; Kaplan Meier (K-M) method was used to describe the survival curve.Results The white blood cell count(WBC) of observation group and control group B were significantly higher than those of control group A (P < 0.05), and the platelet count (PLT) and hemoglobin of observation group and control group B were significantly lower than those of control group A (P < 0.05);there was no significant difference in WBC, PLT, and hemoglobin between the observation group and the control group B (P > 0.05).The expression levels of WT1 and ETS-1 in bone marrow of observation group were (0.83±0.15) and 0.92 (0.22, 1.23), respectively,which were significantly higher than those of control group B (0.35±0.14), 0.48 (0.24, 1.01) and control group A (0.01±0.01) and 0.02(0.01, 0.03) (P < 0.05). The expression levels of WT1 and ETS-1 in bone marrow of control group B were significantly higher than those of control group A (P < 0.05). Logistic regression analysis showed that WT1 and ETS-1 were risk factors for AML. According to the me-dian of WT1 and ETS-1 in AML (non-M3) patients, they were assigned into low expression group and high expression group. K-M showed that the survival rate of low expression group of WT1 and Ets-1 was higher than that of high expression group, and the differ-ence was statistically significant (P < 0.05).Conclusion The expressions of WT1 and ETS-1 in bone marrow of Uygur adults AML (non-M3) patients are high, and the high expression of WT1 and ETS-1 is related to the occurrence of disease and poor prognosis of pa-tients. |
