      目的 探究环状RNA丝氨酸/苏氨酸蛋白扰动样激酶1(Circ TLK1)和微小RNA(miR)-214在缺血性脑卒中(IS)病人中的表达及意义。方法 选取2019年4月至2021年4月在广元市中心医院治疗的97例IS病人为观察组,根据美国国立卫生研究院卒中量表(NIHSS)评分将IS病人分为轻度组33例、中度组45例、重度组19例,另选取85例健康体检者为健康组。采用实时荧光定量逆转录聚合酶链反应(qRT-PCR)检测所有病人血清Circ TLK1和miR-214水平。对IS病人进行90 d的预后随访,按照改良Rankin量表(mRS)分为预后良好组和预后不良组,分析并比较不同预后病人的临床指标。多因素logistic回归分析IS病人预后不良的影响因素。受试者操作特征曲线(ROC曲线)分析Circ TLK1和miR-214对IS病人预后的预测价值。结果与健康组相比,观察组血清Circ TLK1[(1.83±0.22)比(1.03±0.12)]和miR-214[(2.64±0.49)比(1.07±0.13)]均显著性上调(P<0.05)。与轻度组相比,中度组和重度组病人血清Circ TLK1和miR-214均显著性上调,与中度组相比,重度组病人血清CircTLK1和miR-214水平显著上升(P<0.05)。观察组病人90 d预后随访结果显示,预后良好组病人63例,预后不良组病人34例。两组病人同型半胱氨酸(Hcy)、三酰甘油、梗死体积、发病到住院的时间、入院时NIHSS评分、Circ TLK1、miR-214水平之间差异有统计学意义(P<0.05)。多因素logistic回归分析显示,入院时NIHSS评分高、发病到住院的时间长、Circ TLK1、miR-214高水平是IS病人预后不良的独立危险因素(P<0.05)。ROC曲线分析结果显示,Circ TLK1和miR-214对IS病人预后评估价值的曲线下面积(AUC)分别为0.86、0.83,灵敏度分别为91.20%、67.60%,特异度分别为74.60%、90.50%。结论 Circ TLK1和miR-214在IS病人血清中呈高表达,二者是IS病人预后不良的独立危险因素,对病人的预后具有一定的评估价值。

      Objective To explore the expression and significance of circular RNA tousled-like kinase 1 (Circ TLK1) and microRNA(miR)-214 in patients with ischemic stroke (IS).Methods A total of 97 patients with IS treated in Guangyuan Central Hospital from April 2019 to April 2021 were selected as the observation group. According to the National Institutes of Health Stroke Scale (NIHSS) score, IS patients were assigned into a mild group of 33 cases, a moderate group of 45 cases, and a severe group of 19 cases. Another 85 healthy subjects were selected as the healthy group. Real-time fluorescence quantitative reverse transcriptase polymerase chain reac?tion (qRT-PCR) was used to detect the serum Circ TLK1 and miR-214 levels in all patients. The prognosis of IS patients were followed up for 90 days, and they were assigned into a good prognosis group and a poor prognosis group according to the modified Rankin scale (mRS), the clinical indicators of patients with different prognosis were analyzed and compared. Multivariate logistic regression was used to analyze the factors affecting the poor prognosis of IS patients. The receiver operating characteristic (ROC) curve was used to analyze the predictive value of Circ TLK1 and miR-214 on the prognosis of IS patients.Results Compared with the healthy group, the serum Circ TLK1 [(1.83±0.22) vs. (1.03±0.12)] and miR-214 [(2.64±0.49) vs. (1.07±0.13)] in the the observation group were significantly upregulated,and the difference was statistically significant (P<0.05). Compared with the mild group, the serum Circ TLK1 and miR-214 in the moderate group and the severe group were significantly up-regulated, compared with the moderate group, the serum Circ TLK1 and miR-214 levels in the severe group were significantly increased, and the difference was statistically significant (P<0.05). The 90-day prognosis follow-up results of the observation group showed that there were 63 patients in the good prognosis group and 34 patients in the poor prognosis group. There were significant differences between the two groups in homocysteine (Hcy), triglycerides, infarct vol?ume, time from onset to hospitalization, NIHSS score on admission, Circ TLK1 and miR-214 levels (P<0.05). Multivariate logistic re?gression analysis showed that high NIHSS score on admission, long time from onset to hospitalization, high levels of Circ TLK1 and miR-214 were independent risk factors for poor prognosis of IS patients (P<0.05). ROC curve analysis results showed that the area un?der the curve (AUC) of Circ TLK1 and miR-214 for the prognostic evaluation of IS patients was 0.86 and 0.83, respectively, the sensitiv?ity was 91.20% and 67.60%, respectively, and the specificity was 74.60% and 90.50%, respectively.Conclusions CircTLK1 and miR-214 are highly expressed in the serum of IS patients. Both of them are independent risk factors for poor prognosis of IS patients,and have certain evaluation value for the prognosis of patients.
