      目的通过多节点观察早产儿在矫正 24月龄内发育进程情况及情绪行为问题,为指导早产儿家庭科学养育照护提供理论依据。方法回顾性调查安徽省妇幼保健院儿童保健科自 2014年 4月至 2021年 10月常规健康体检的早产儿 343例,按年龄分组,并按 1∶2比例选择同期参与健康体检的足月儿 686例,采用《年龄与发育进程问卷 -3》对研究对象发育进程进行评估,同时访谈综合问题各项。结果早产儿组于矫正 12~16月龄时除粗大动作外,沟通、精细动作、解决问题和个人 -社会能区均存在落后情况( P<0.05);在矫正 >16~20月龄时解决问题能区已追上足月儿( 52.33±10.25比 53.12±9.64);在矫正 >20~24月龄时早产儿组个人 -社会能仍落后于足月儿( 43.99±9.57比 51.21±8.72)。综合访谈结果显示,早产儿父母在其语言发育与情绪行为问题方面的困扰明显多于足月儿父母(分别为 20.70%比 12.97%,28.86%比 8.02%)。结论早产儿早期发育进程存在追赶不均衡现象,其凸显的社会适应性和情绪行为问题应引起广大医务人员的关注。
英文摘要:
      Objective To provide a theoretical basis for guiding preterm infants′ family scientific nurturing and care, a multiple cross-sectional research of their development process and emotional problems was conducted.Methods A retrospective study was conducted on 343 premature infants who underwent routine physical examination in the Child Health Department of Anhui Maternal andChild Health Hospital from April 2014 to October 2021, and 686 full-term infants who participated in physical examination at the same period were selected as control group according to 1:2 ratio. Age and Stages Questionnaries-3(ASQ-3) was used to screen the intelligent and other problems of the subjects.Results At correction age of 12 to 16 months, except for gross motor, premature infants lagged behind in communication, fine motor, problem solving ability and personal-social ability, compared with full-term ones (P<0.05); at >16 to 20 months of age, they had caught up in problem solving ability (52.33±10.25 vs. 53.12±9.64 ); at corrected >20 to 24 months, preterm group still lagged behind in terms of personal-social area (43.99±9.57 vs. 51.21±8.72) . Besides, parents of premature infants had moremisgivings about their language development and emotional behavior problems than parents of full-term ones (20.70% vs. 12.97%, 28.86% vs. 8.02%).Conclusion Preterm infants have a good overall catch-up trend, but their social adaptability and emotional behavior problems should be taken into consideration.
