      目的探究急性缺血性脑卒中( AIS)治疗前血清微 RNA(miR)-497的浓度对预测 AIS病人治疗后出血转化( HT)事件的价值。方法收集邯郸市第二医院神经内科院确诊的 152例 AIS病人,静脉溶栓后根据是否发生 HT事件分为 HT组和非 HT组,通过 RNA测序检测两组病人治疗前的 miRNA表达谱,统计 AIS病人治疗前的基线和临床资料,采用 logistic回归分析 HT的危险因素。通过实时定量 PCR检测 miR-497在 AIS病人血清中的浓度,并采用受试者操作特征曲线( ROC)评价 miR-497预测 HT事件的价值,此外利用 miRTarBase V8.0预测 miR-497的靶 mRNA,应用京都基因和基因组百科全书( KEGG)分析 miR-497潜在的高级通路功能。结果本研究共纳入 152例 AIS病人,采用静脉溶栓后 7d内共有 28例发生 HT事件,其中 HT组的空腹血糖( FBG)水平、高血压病史、糖尿病史、 NIHSS评分以及大动脉粥样硬化型与非 HT组病人相比差异有统计学意义,是发生 HT事件的危险因素。 RNA测序结果表明 HT病人治疗前与非 HT病人之间存在 9个差异 miRNAs,其中相比非 HT组病人, miR-497在 HT组病人显著升高,同时 ROC结果提示 miR-497预测 HT的线下面积为 0.93,灵敏度为 86.4%,特异度为 82.3%,最佳截断值为 1.52 μg/L。功能富集分析则表明 miR-497可靶向调控 101个 mRNA,并通过影响大脑炎性反应、氧化应激以及神经元自噬等途径参与 AIS和 HT的发生。结论 AIS病人治疗前血清 miR-497显著升高,对预测治疗过程中 HT事件具有较高的临床价值。

      Objective To investigate the value of serum microRNA (miR) -497 concentration before treatment of acute ischemic stroke (AIS) in predicting hemorrhagic transformation (HT) events in patients with AIS after treatment.Methods One hundred and fif-ty-two AIS patients confirmed by the Department of Neurology of the Second Hospital of Handan were continuously collected. After in-travenous thrombolysis, they were assigned into HT group and non-HT group according to whether HT events occurred. miRNA expres-sion profiles of the two groups before treatment were detected by RNA sequencing, and the baseline and clinical data of AIS patients be-fore treatment were analyzed. Risk factors of HT were analyzed by logistic regression. Real-time quantitative PCR was used to detect the serum concentration of miR-497 in AIS patients, and receiver Operator characteristic curve (ROC) was used to evaluate the value of miR-497 in predicting HT events. In addition, the target mRNA of miR-497 was predicted by miRTarBase V8.0. The potential higher pathway functions of miR-497 were analyzed using the Kyoto Encyclopedia of Genes and Genomes (KEGG).Results A total of 152 AIS patients were included in this study, and 28 patients developed HT events within 7 days after intravenous thrombolytic therapy.FBG level, history of hypertension, history of diabetes and NIHSS score and large atherosclerotic type in THE HT group were signifi-cantly different from those in the non-HT group, which were risk factors for HT events. RNA sequencing results showed that there were9 different miRNAs between HT patients and non-HT patients before treatment, among which, miR-497 significantly increased in HT patients compared with non-HT patients. Meanwhile, ROC results suggested that miR-497 predicted the Area Under Curve of HT witha sensitivity of 0.93, a specificity of 86.4%, and a specificity of 82.3%. The optimal truncation value was 1.52 μg/L. Functional enrich-ment analysis showed that miR-497 could target 101 mRNA and participate in the occurrence of AIS and HT by affecting the brain in-flammatory response, oxidative stress and neuronal autophagy.Conclusion Serum miR-497 is significantly increased in AIS patients before treatment, which has high clinical value in predicting HT events during treatment.
