      目的探究 Y染色体性别决定区相关高速泳动族框因子 9(SOX9)和周期素依赖性激酶 4(CDK4)在结直肠息肉、结直肠癌中的表达及意义。方法选取中国人民解放军第八十二集团军医院 2015年 1月至 2017年 6月收治的结直肠癌病人病理组织标本(结直肠癌组) 94例及癌旁正常结直肠组织(正常结直肠组) 39例,另取同时期结直肠息肉病人结直肠息肉组织(结直肠息肉组) 51例。采用免疫组织化学 SP染色法检测 SOX9、CDK4的表达情况,分析 SOX9、CDK4表达与结直肠癌病人临床病理参数之间的关系;对结直肠癌病人进行术后复查随访,分析结直肠癌病人生存情况。结果 SOX9在正常结直肠组织、结直肠息肉组织和结直肠癌组织中的阳性表达率分别为 10.26%(4/39)、 27.45%(14/51)、 76.59%(72/94),差异有统计学意义( P<0.05); CDK4在正常结直肠组织、结直肠息肉组织和结直肠癌组织中的阳性表达率分别为 7.69%(3/39)、 21.57%(11/51)、54.84%(51/94),差异有统计学意义( P<0.05);正常结直肠组织中 SOX9、CDK4蛋白表达水平低于结直肠息肉组织、结直肠癌组织( P<0.05),结直肠息肉组织中 SOX9、CDK4蛋白表达水平低于结直肠癌组织( P<0.05)。结直肠癌病人病理组织 SOX9、CDK4表达与肿瘤分化程度、临床分期、是否合并淋巴结转移有关( P<0.05);结直肠癌组织 SOX9与 CDK4表达呈正相关( r=0.58,P<0.001)由TCGA数据库检索结直肠癌组织 SOX9、CDK4基因表达相关性显示,二者之间亦呈正相关( r=0.40,P<0.001); SOX9高表达、低表,达病人术后 3年累积生存率分别为 44.6%、72.4%,两组比较 Log-rank检验差异有统计学意义( χ2=8.31,P=0.004); CDK4高表达、低表达病人术后 3年累积生存率分别为 25.6%、76.5%,两组比较 Log-rank检验差异有统计学意义( χ2=36.36,P<0.001)。结论 SOX9和 CDK4在结直肠癌组织、结直肠息肉组织、正常结直肠组织中的阳性表达率逐渐降低,与结直肠癌肿瘤分化程度、临床分期、淋巴结转移情况及预后有关,可能参与结直肠癌的发生发展过程。

      Objective To investigate the expression and significance of sex-determining region Y (SRY)-box 9 protein (SOX9) and cyclin-dependent kinase 4 (CDK4) in colorectal polyps and colorectal cancer.Methods Ninety-four cases of pathological tissue specimens of colorectal cancer patients and 39 cases of adjacent normal colorectal tissue treated in The 82nd Group Military Hospital of thePeople′s Liberation Army from January 2015 to June 2017 were selected, and another 51 cases of colorectal polyps were taken from patients with colorectal polyps at the same time. The expression of SOX9 and CDK4 was detected by immunohistochemical SP staining,the relationship between the expression of SOX9 and CDK4 and clinicopathological parameters was analyzed; the patients with colorectal cancer were followed up after operation to analyze the survival of patients with colorectal cancer.Results The positive expressionrate of SOX9 in normal colorectal tissues, colorectal polyps and colorectal cancer was 10.26% (4/39), 27.45% (14/51) and 76.59% (72/94), respectively, and the difference was statistically significant (P<0.05). The positive expression rate of CDK4 in normal colorectal tissues, colorectal polyps and colorectal cancer was 7.69% (3/39), 21.57% (11/51) and 54.84 % (51/94), respectively, and the differencewas statistically significant (P<0.05). The expression levels of SOX9 and CDK4 protein in normal colorectal tissues were lower thanthose in colorectal polyps and colorectal cancer, and the difference was statistically significant (P<0.05), and the expression levels of SOX9 and CDK4 protein in colorectal polyps were lower than those in colorectal cancer, and the difference was statistically significant(P<0.05). The expression of SOX9 and CDK4 in colorectal cancer was correlated with tumor differentiation, clinical stage and lymphnode metastasis (P<0.05). There was a positive correlation between SOX9 and CDK4 expression in colorectal cancer (r=0.58, P<0.001),and there was a positive correlation between SOX9 and CDK4 gene expression in colorectal cancer retrieved from TCGA database (r= 0.40, P<0.001). The 3-year cumulative survival rates of patients with high and low SOX9 expression were 44.6 % and 72.4 %, respectively, and there was significant difference between the two groups in Log-rank test (χ2=8.31, P=0.004). The 3-year cumulative survivalrates of patients with high and low CDK4 expression were 25.6 % and 76.5 %, respectively, and there was significant difference in Log-rank test between the two groups (χ2=36.36, P<0.001).Conclusion The positive expression rates of SOX9 and CDK4 in colorectal cancer, colorectal polyps and normal colorectal tissues are gradually decreased, which are related to tumor differentiation, clinical stage,lymph node metastasis and prognosis of colorectal cancer, and may participate in the occurrence and development of colorectal cancer.
