      目的探讨微 RNA-125a-5p(miR-125a-5p)和维生素 D受体( VDR)在乙型肝炎病毒( HBV)相关肝细胞癌(肝癌)病人组织中的表达情况及临床意义。方法选取 2014年 2月至 2016年 5月在沧州市第三医院接受手术治疗的 HBV相关肝癌病人 65例,术中取病人肝癌组织及癌旁组织,比较 miR-125a-5p、VDR mRNA在不同肝组织中的表达差异,分析 miR-125a-5p、VDR mRNA水平与肝癌病人临床病理特征的关系,采用 Pearson法分析 miR-125a-5p、VDR mRNA的相关性。随访 5年, Kaplan-Meier法分析 miR-125a-5p、VDR mRNA水平对肝癌病人生存率的影响。结果肝癌组织 miR-125a-5p水平 0.69±0.16低于癌旁组织0.97±0.11(P<0.05),VDR mRNA水平 1.73±0.45高于癌旁组织 1.03±0.12(P<0.05)。 miR-125a-5p、VDR mRNA水平与 HBV相关肝癌病人门静脉侵袭、淋巴结转移、分化程度及 TNM分期关系密切( P<0.05)。肝癌病人肝癌组织中 miR-125a-5p与 VDR mRNA呈负相关( r=?0.44,P<0.001)。 miR-125a-5p高表达组 5年生存率为 65.63%,高于 miR-125a-5p低表达组的 39.39%(P<0.05); VDR mRNA高表达组 5年生存率为 37.50%,低于 VDR mRNA低表达组的 66.67%(P<0.05)。 miR-125a-5p高表达 +VDR mRNA低表达病人、 miR-125a-5p低表达 +VDR mRNA高表达病人、 miR-125a-5p高表达 +VDR mRNA高表达病人、 miR-125a-5p低表达 +VDR mRNA低表达病人 5年生存率分别为 69.57%、23.81%、63.64%、60.00%,四组比较差异有统计学意义( P<0.05)。结论 HBV相关肝癌病人肝癌组织中 miR-125a-5p表达降低, VDR mRNA水平升高,二者与门静脉侵袭、淋巴结转移、分化程度及 TNM分期有关, miR-125a-5p高表达及 VDR mRNA低表达病人 5年生存率较高,对于预后评估有一定价值。

      Objective To investigate the expression and clinical significance of microRNA-125a-5p (miR-125a-5p) and vitamin D receptor (VDR) in the tissues of hepatitis B virus (HBV)-related hepatocellular carcinoma (liver cancer) patients.Methods From February 2014 to May 2016, Sixty-five HBV-related liver cancer patients who underwent surgical treatment in Cangzhou Third Hospitalwere selected. Liver cancer tissues and para-cancerous tissues were collected from the patients during the operation, and the survivalstatus of the patients was followed up for 5 years. The expression differences of miR-125a-5p and VDR mRNA in different liver tissues were compared, the relationship between miR-125a-5p, VDR mRNA levels and clinicopathological characteristics of liver cancer patients was analyzed, the correlation between miR-125a-5p and VDR mRNA was analyzed with Pearson method, the effects of miR-125a5p and VDR mRNA levels on the survival rate of liver cancer patients were analyzed with Kaplan-Meier method.Results The level of miR-125a-5p 0.69±0.16 in liver cancer tissues was lower than that of adjacent tissues 0.97±0.11 (P<0.05), and the level of VDR mRNA 1.73±0.45 was higher than that of adjacent tissues 1.03±0.12 (P<0.05). The levels of miR-125a-5p and VDR mRNA were closely related to portal vein invasion, lymph node metastasis, degree of differentiation and TNM stage in patients with HBV-related liver cancer (P<0.05). There was a negative correlation between miR-125a-5p and VDR mRNA in liver cancer tissues of patients with liver cancer (r= ? 0.44, P<0.001). The 5-year survival rate of miR-125a-5p high expression group was 65.63%, which was higher than39.39 % of miR-125a-5p low expression group (P<0.05); The 5-year survival rate of the VDR mRNA high expression group was37.50%, which was lower than the 66.67% of the VDR mRNA low expression group (P<0.05). The 5-year survival rates of the patients with high expression of miR-125a-5p + low expression of VDR mRNA, patients with low expression of miR-125a-5p + high expression of VDR mRNA, patients with high expression of miR-125a-5p + high expression of VDR mRNA, patients with low expression of miR125a-5p + low expression of VDR mRNA were 69.57%, 23.81%, 63.64%, and 60.00%, respectively, the differences among the fourgroups were statistically significant (P<0.05). Conclusions The expression of miR-125a-5p in liver cancer tissues of patients with HBV-related liver cancer is decreased, and the level of VDR mRNA is increased, both of which are related to portal vein invasion,lymph node metastasis, degree of differentiation and TNM stage. Patients with high expression of miR-125a-5p and low expression of VDR mRNA have a higher 5-year survival rate, which is of certain value for prognostic evaluation.
