      目的探讨长链非编码 RNA(LncRNA)MicroRNA155宿主基因( MIR155HG)、微 RNA(miR)-128-5p水平与慢性阻塞性肺疾病( COPD)病人的关系及临床意义。方法选取 2019年 5月至 2021年 5月北京市第一中西医结合医院收治的 COPD病人 100例,其中急性加重期 COPD病人为 A组( n=52)稳定期 COPD病人为 B组( n=48),选取同期来该院体检且与病人一般资料匹配的健康人群为C组(n=50)。采用实时荧光定量,PCR(qRT-PCR)法检测血清 LncRNA MIR155HG、miR-128-5p表达水平; Pearson法分析 COPD病人血清 LncRNA MIR155HG与 miR-128-5p表达水平的相关性;受试者操作特征( ROC)曲线分析血清 LncRNA MIR155HG、miR-128-5p水平诊断急性加重期 COPD病人的价值。结果 COPD加重期和稳定期病人血清中 LncRNA MIR155HG(3.59±0.82、2.35±0.55)与 miR-128-5p(0.33±0.11、0.51±0.12)的表达相较于健康人群( 1.06±0.27、0.95±0.13)中升高,差异有统计学意义(F=230.26、356.75,P<0.05); COPD病人血清中肿瘤坏死因子 -α(TNF-α)、降钙素原、超敏 C反应蛋白(hsCPR)、白细胞水平均上升,第一秒用力呼吸容积(FEV1)%、FEV1/用力肺活量(FVC)水平均下调,差异有统计学意义(P<0.05);清中 LncRNA MIR155HG与 TNF-α、降钙素原、 hs-CPR、白细胞呈正相关,与 FEV1%、FEV1/FVC呈负相关(P<0.05); miR-128-5p与血TNF-α、降钙素原、 hs-CRP、白细胞呈负相关,与 FEV1%、FEV1/FVC呈正相关( P<0.05); COPD病人血清 LncRNA MIR155HG和 miR-128-5p表达呈明显负相关( r=?0.49,P<0.05); ROC结果显示, LncRNA MIR155HG、miR-128-5p二者联合诊断急性加重期 COPD病人的曲线下面积( AUC)为 0.98,95%CI:(0.93,1.00),显著高于 LncRNA MIR155HG、miR-128-5p单独诊断。结论 LncRNA MIR155HG在 COPD病人血清中明显高表达, miR-128-5p呈低表达,二者呈明显负相关,且与病情严重程度密切相关,二者联合检测对评估急性加重期 COPD病人的临床价值高于单一指标,可作为诊断急性加重期 COPD病人的潜在血清标志物。

      Objective To investigate the relationship between and clinical significance of the long noncoding RNA (lncRNA) MIR155HG and miR-128-5p levels in patients with chronic obstructive pulmonary disease (COPD).Methods A total of 100 COPD patients admitted to Beijing First Integrated Traditional Chinese and Western Medicine Hospital from May 2019 to May 2021 were selected. Among them, patients with acute exacerbation COPD were group A (n=52), patients with stable COPD were group B (n=48), andhealthy people who came to our hospital for physical examination during the same period and matched with patients′ general information were selected as group C (n=50). Quantitative real-time fluorescent quantitative PCR (qRT?PCR) was used to detect the expression levels of serum lncRNA MIR155HG and miR-128-5p. Pearson′s method was used to analyze the correlation between the expression levels of serum lncRNA MIR155HG and miR-128-5p in COPD patients. Receiver operating characteristic curve (ROC) analysis of the value of serum lncRNA MIR155HG and miR-128-5p levels in diagnosing patients with acute exacerbation of COPD.Results The expressions levels of lncRNA MIR155HG (3.59±0.82, 2.35±0.55) and miR-128-5p (0.33±0.11, 0.51±0.12) were elevated in the serum of exacerbation and stable COPD patients compared to the healthy population (1.06±0.27, 0.95±0.13), with statistically significant differences( F=230.26, 356.75,P<0.05). The serum levels of tumor necrosis factor-α (TNF-α), calcitoninogen (PCT), hypersensitive C-reactive protein (hs-CPR), and white blood cells (WBC) were increased, and the levels of forced expiratory volume in one second (FEV1) % and FEV1/forced vital capacity (FVC) were downregulated in COPD patients, with statistically significant differences (P<0.05). The serum lncRNA MIR155HG was positively correlated with TNF-α, PCT, hs-CPR, and WBC count, and negatively correlated with FEV1% and FEV1/FVC (P<0.05), while miR-128-5p was negatively correlated with TNF-α, PCT, hs-CRP, and WBC count, and positively correlated with FEV1% and FEV1/FVC (P<0.05). The expression of serum lncRNA MIR155HG and miR-128-5p in COPD patients was significantly negatively correlated (r=?0.49, P<0.05). The ROC results showed that the area under the curve (AUC) of LncRNA MIR155HG and miR-128-5p were significantly higher than those of lncRNA MIR155HG and miR-128-5p alone in the combined diagnosis of COPD patients with acute exacerbation stage at 0.98, 95%CI: (0.93, 1.00). Conclusions LncRNA MIR155HG was significantly highly expressed and miR-128-5p was lowly expressed in the serum of COPD patients, and the two were significantly negatively correlated andclosely related to the severity of the disease. The combination of the two tests is of higher clinical value than a single index in the assessment of patients with acute exacerbation of COPD and can be used as a potential serum marker for the diagnosis of patients with acuteexacerbation of COPD.
