目的探讨金黄色葡萄球菌( SA)对万古霉素的耐药性及耐药基因分布情况。方法收集平顶山市第一人民医院 2020年 4月至 2022年 3月临床送检样本分离的 288株 SA菌株,采用 Vitek 2 Compact全自动细菌鉴定和药敏系统以及纸片扩散法进行菌落鉴定与药敏试验;采用聚合酶链反应检测 pbp4、mgrA、agr、vraS、vraR、icaA、icaR基因表达。结果 288株 SA菌株中筛选出 25株万古霉素异质性耐药金黄色葡萄球菌( hVISA)菌株, hVISA阳性率为 8.68%,未检测到万古霉素中介耐药金黄色葡萄球菌(VISA)菌株和万古霉素耐药金黄色葡萄球菌( VRSA)菌株;万古霉素敏感金黄色葡萄球菌(VSSA)菌株万古霉素最小抑菌浓度( MIC)主要分布在 0.50 mg/L和 0.75 mg/L,占比为 41.83%和 28.90%;hVISA菌株万古霉素 MIC主要分布在 1.50 mg/L和 2.00 mg/L,占比为 56.00%和 28.00%;VSSA菌株 pbp4、agr、icaR的相对表达量均低于 hVISA菌株( P<0.05)而 mgrA、vraS、vraR、 icaA的相对表达量均高于 hVISA菌株( P<0.05)。结论本次调查 hVISA两年总流行率为 8.68%。与 VSSA株比较, hVISA菌菌,株 pbp4、agr、icaR、mgrA、vraS、vraR、icaA基因表达存在差异,可能与万古霉素的耐药性有关。 |

Objective To investigate the drug resistance of Staphylococcus aureus (SA) to vancomycin and the distribution of drug resistance genes.Methods A total of 288 strains of SA isolated from clinical samples submitted in Pingdingshan First People′s Hospitalfrom April 2020 to March 2022 were collected. Vitek 2 Compact automatic bacterial identification, Drug Sensitivity System and disk diffusion method were used for colony identification and drug sensitivity test. The gene expressions of pbp4, mgrA, agr, vraS, vraR, icaA andicaR were detected by polymerase chain reaction.Results Among 288 SA strains, 25 vancomycin heterogeneous resistant staphylococcus aureus (hVISA) strains were screened, and the positive rate of hVISA was 8.68 %. Vancomycin mediated resistance staphylococcusaureus (VISA) strains and vancomycin resistant staphylococcus aureus (VRSA) strains were not detected. Vancomycin minimum inhibitory concentration (MIC) of vancomycin sensitive staphylococcus aureus (VSSA) strains were mainly distributed at 0.50 mg/L and 0.75 mg/L, accounting for 41.83 % and 28.90 %. The vancomycin MIC of hVISA strains were mainly distributed at 1.50 mg/L and 2.00 mg/L, accounting for 56.00 % and 28.00 %. The relative expressions of pbp4, agr and icaR in VSSA strains were lower than those in hVISA strains(P<0.05), while the relative expressions of mgrA, vraS, vraR and icaA were higher than those in hVISA strains (P<0.05). Conclusion The prevalence of hVISA in SA in our hospital in recent two years is 8.68 %. There are differences in the expressions of pbp4, mgrA, agr,vraS, vraR, icaA and icaR genes between VSSA and hVISA strains, which may be related to the drug resistance of vancomycin. |
