      目的:分析血清趋化因子样受体1(CMKLR1)、几丁质酶1(CHIT1)水平是否与妊娠糖尿病患者胰岛素抵抗有关。方法:选取2019年1月至2022年1月在本院诊治的204例妊娠糖尿病孕妇作为研究组,另选取同期孕检健康孕妇204例为对照组;血清CMKLR1、CHIT1水平采用酶联免疫吸附法(ELISA)测定;采用Pearson法分析血清CMKLR1、CHIT1相关性以及二者与胰岛素抵抗相关指标的相关性;Logistic回归分析影响妊娠糖尿病的因素。结果:研究组空腹血糖(FPG)、餐后30 min,1 h,2 h血糖(30 min PG,1hPG,2hPG)、糖化血红蛋白(HbA1c)、空腹胰岛素(FINS)、餐后30 min,1 h,2h胰岛素(30 min INS,1 h INS,2h INS)、甘油三酯(TG)、血清CMKLR1、CHIT1水平、胰岛素抵抗指数(HOMA-IR)显著高于对照组(P<0.05),胰岛素β细胞功能指数(HOMA-β)以及早期胰岛素分泌指数(△I30/△G30)显著低于对照组(P<0.05)。根据pearson相关性分析得知,血清CMKLR1、CHIT1水平呈正相关(P<0.05),血清CMKLR1、CHIT1水平与HOMA-IR呈正相关(P<0.05),与HOMA-β、△I30/△G30呈负相关(P<0.05)。行Logistic回归分析得知,CMKLR1、CHIT1以及HOMA-IR高水平,HOMA-β和△I30/△G30低水平是影响妊娠糖尿病的危险因素(P<0.05)。结论:妊娠糖尿病患者血清CMKLR1、CHIT1水平显著升高,且与胰岛素抵抗密切相关。

      Objective: Serum chemokine-like receptor 1 (CMKLR1) and chitinase 1 (CHIT1) levels were analyzed for insulin resistance in gestational diabetics. Methods: A total of 204 pregnant women with gestational diabetes diagnosed and treated in our hospital from January 2019 to January 2022 were collected as the study group, another 204 healthy pregnant women in the same period were collected as the control group;Serum CMKLR1 and CHIT1 levels were determined by enzyme-linked immunosorbent assay (ELISA); Pearson"s method was applied to analyze the correlation between serum CMKLR1, CHIT1, and insulin resistance related indicators; Logistic regression analyzes factors affecting gestational diabetes. Results: Fasting blood glucose (FPG), 30 min after prandial, 1 h, 2 h blood glucose (30 min PG, 1 hPG, 2 hPG), glycated hemoglobin (HbA1c), fasting insulin (FINS), 30 min, 1 h, 2 h insulin (30 min INS, 1 h INS, 2h INS), triglycerides (TG), serum CMKLR1, CHIT1 levels, The insulin resistance index (HOMA-IR) was significantly higher than that of the control group (P<0.05), and the insulin β cell function index (HOMA-β) and early insulin secretion index (△I30/△G30) were significantly lower than that of the control group (P<0.05). According to Pearson correlation analysis, serum levels of CMKLR1 and CHIT1 were positively correlated (P<0.05), while serum levels of CMKLR1 and CHIT1 were positively correlated with HOMA-IR (P<0.05), and negatively correlated with HOMA-β and △I30/△G30 (P<0.05). Logistic regression analysis showed that high levels of CMKLR1, CHIT1 and HOMA-IR, and low levels of HOMA-β and △I30/△G30 were risk factors for gestational diabetes (P<0.05). Conclusion: The serum levels of CMKLR1 and CHIT1 in patients with gestational diabetes are obviously elevated and closely related to insulin resistance.
