目的测定新疆维吾尔族异基因造血干细胞移植( hematopoietic stem cell,HSCT)病儿血浆中伏立康唑浓度。方法采用高效液相色谱法 -质谱联用技术测定接受伏立康唑治疗的 204例异基因 HSCT病儿的伏立康唑谷浓度。结果血浆样品中伏立康唑与 IS的峰面积比在 0.2~20 mg/L浓度范围内呈线性( Y=1.107 2 X?0.051 3,r2 ≥ 0.999 7)。定量下限( LLOQ)、低浓度质控( LQC)、中浓度质控( MQC)和高浓度质控( HQC)的日内、日间精密度的 CV值小于 7.39%,准确度在理论浓度的 ±15%内,提取回收率为 85.46%~95.55%,标准偏差均 <5.0%,室温、短期、长期和冻融冻存稳定性均较好( CV值均小于 10%)。 9例病儿的伏立康唑血浆谷浓度 <0.5 mg/L,26例伏立康唑谷浓度 > 6 mg/L。结论该方法特异性强、灵敏度高、运行时间短,能满足临床伏立康唑血药浓度测定需求。 |

Objective Determination of voriconazole concentration in plasma of high-risk children with hematopoietic stem cell transplantation (HSCT) of Xinjiang Uygur.Methods High-performance liquid chromatography-mass spectrometry was used to monitorvoriconazole trough concentrations in 204 children with allogeneic HSCT in hematology who received voriconazole.Results The peak area ratio of voriconazole to IS in plasma samples was linear over the concentration range of 0.2-20 mg/L (Y=1.107 2 X?0.051 3, r2≥0.999 7). The CV (%) values of intra-day and inter-day precision for LLOQ, LQC, MQC and HQC were not more than 7.39%, the accuracy was within ±15% of the theoretical concentration, and the extraction recovery was 85.46% to 95.55%, the standard deviation wereall < 5.0%, and the room temperature, short-term, long-term and freeze-thaw storage stability were all good (CV values were all lessthan 10%). The plasma trough concentration of voriconazole were < 0.5 mg/L in nine children, and 26 children were > 6 mg/L.Conclu? sion The method has high analytical sensitivity, strong specificity and short running time, which can meet the needs of clinical voriconazole concentration monitoring. |
