| 摘要：目的 探索安罗替尼、卡瑞利珠单抗联合化疗治疗肺肉瘤样癌的治疗经验。方法 对潍坊市人民医院2023年3月收治的1例肺肉瘤样癌患者临床资料及肺肉瘤样癌临床特点进行分析、总结。结果 患者肺部多发结节入院，入院后行强化CT示双肺多发占位，考虑恶性，右肺恶性肿瘤并双肺转移。右肺占位穿刺病理结果示梭形细胞肿瘤，符合肉瘤。诊断为右肺上叶恶性肿瘤（肉瘤，T4N3M1，IV期）。行安罗替尼、卡瑞利珠单抗联合化疗方案治疗。治疗后复查胸部CT示病灶减小，按实体瘤疗效评定标准(RECIST 1.1）评估，患者疗效评估为部分缓解(Partial Response，PR)。结论 肺肉瘤样癌患者经安罗替尼、卡瑞利珠单抗联合化疗治疗后，患者疗效评估为PR。表明化疗结合抗血管新生及免疫治疗可使肺肉瘤样癌（pulmonary sarcomatoid carcinoma，PSC）患者受益。 |

| Abstract: Objective To explore the treatment experience of anlotinib and camrelizumab combined with chemotherapy in the treatment of pulmonary sarcomatoid carcinoma. Methods The clinical data and clinical characteristics of a patient with pulmonary sarcomatoid cancer admitted to Weifang People's Hospital in March 2023 were analyzed and summarized. Results The patient was admitted to the hospital with multiple nodules in the lungs. After admission, enhanced CT showed multiple space-occupying lesions in both lungs, which was considered malignant. The patient had a malignant tumor in the right lung and metastasis to both lungs. The pathological results of right lung mass puncture showed spindle cell tumor, consistent with sarcoma. A malignant tumor (sarcoma, T4N3M1, stage IV) was diagnosed in the upper lobe of the right lung. The patient was treated with anlotinib and camrelizumab combined with chemotherapy regimen. Chest CT reexamination after treatment showed that the lesions had decreased. According to the Response Evaluation Criteria for Solid Tumors (RECIST 1.1), the patient's response was evaluated as partial response (PR). Conclusion After patients with pulmonary sarcomatoid carcinoma were treated with anlotinib and camrelizumab combined with chemotherapy, the patient's efficacy was evaluated as PR. It shows that chemotherapy combined with anti-angiogenesis and immunotherapy can benefit patients with pulmonary sarcomatoid carcinoma (PSC). |
