目的探究重症急性胰腺炎( SAP)病人血清微 RNA-143-3p(miR-143-3p)、环氧合酶 -2(COX-2)表达水平与辅助性 T细胞 17(Th17)/调节性 T细胞( Treg)平衡的关系。方法选取 2017年 12月至 2020年 9月榆林市第一医院诊治的 103例急性胰腺炎( AP)病人进行研究,按 APACHEⅡ评分将其分为 SAP组( 45例)和轻症急性胰腺炎( MAP)组( 58例)另选取同期 55例体检健康者为健康组。比较各组血清 miR-143-3p、维甲酸相关孤核受体 γt(RORγt)mRNA、COX-2 mRNA、叉头,/翼状螺旋转录因子 3(Foxp3)mRNA、Th17、Treg、Th17/Treg水平;分析 SAP病人血清 miR-143-3p、COX-2 mRNA表达水平与外周血 Th17、Treg、 Th17/Treg、RORγt mRNA、Foxp3 mRNA的相关性;分析 SAP发生的影响因素。结果 MAP组、 SAP组病人血清 miR-143-3p(0.70±0.23、0.42±0.14比 1.04±0.35)、 Foxp3 mRNA表达水平( 0.56±0.19、0.31±0.11比 1.03±0.34)及 Treg细胞百分比[( 6.13±2.04)%、(2.73±0.90)%比( 8.74±2.91)%]低于健康组( P<0.05)COX-2 mRNA(1.68±0.48、2.37±0.54比 1.01±0.34)、 RORγt mRNA表达水平( 1.72±0.57、2.44±0.81比 1.05±0.35)及 Th17细胞比[( 3.87±1.29)%、(6.71±2.34)%比( 1.99±0.66)%]、 Th17/Treg百分,(0.63±0.21、2.46±0.82比 0.23±0.08)高于健康组( P<0.05); SAP组病人血清 miR-143-3p、Foxp3 mRNA表达水平及 Treg细胞百分比低于 MAP组( P<0.05)COX-2 mRNA、RORγt mRNA表达水平及 Th17细胞百分比、 Th17/Treg高于 MAP组( P<0.05); SAP病人血清 miR-143-3p表达水与 Th17、RORγt mRNA、Th17/Treg及血清 COX-2 mRNA表达水平与 Treg、Foxp3 mRNA呈负相关( P<0.05)血清 miR-143-3p表达水平与 Treg、Foxp3 mRNA及血清 COX-2 mRNA表达水平与 Th17、RORγt mRNA、Th17/Treg呈正相关( P<0.,05); SAP病人血清 miR-143-3p表达水平与 COX-2 mRNA呈负相关( P<0.05); miR-143-3p是影响 SAP发生的保护因素(P<0.05),COX-2 mRNA、Th17/Treg是影响 SAP发生的危险因素( P<0.05)。结论 SAP病人血清 miR-143-3p表达水平较低, COX-2 mRNA表达水平较高,二者均与 Th17/Treg平衡相关, miR-143-3p可能为治疗 SAP的新靶点。 |

Objective To explore the relationship between the expression levels of serum microRNA-143-3p (miR-143-3p) and cyclooxygenase-2 (COX-2) and helper T cell 17 (Th17)/regulatory T cell (Treg) balance in patients with severe acute pancreatitis (SAP). Methods A total of 103 patients with acute pancreatitis (AP) who were diagnosed and treated in Yulin First Hospital from December2017 to September 2020 were selected for the study. They were divided into SAP group (45 cases) and mild AP (MAP) group (58 cases)according to the APACHEⅡ score, another 55 healthy persons undergoing physical examination during the same period were selectedas the healthy group. The levels of serum miR-143-3p, retinoic acid-related orphan receptor γt (RORγt) mRNA, COX-2 mRNA, forkhead or winged helix transcription 3 (Foxp3) mRNA, Th17, Treg, and Th17/Treg in each group were compared; the correlation betweenserum miR-143-3p, COX-2 mRNA expression levels and peripheral blood Th17, Treg,Th17/Treg, RORγt mRNA, and Foxp3 mRNA inSAP patients was analyzed; the influencing factors of SAP were analyzed.Results The expression levels of serum miR-143-3p [(0.70± 0.23), (0.42±0.14) vs. (1.04±0.35)] and Foxp3 mRNA [(0.56±0.19), (0.31±0.11) vs. (1.03±0.34)] and the percentage of Treg cells [(6.13± 2.04)%, (2.73±0.90)% vs. (8.74±2.91) %] in the MAP group and SAP group were lower than those in the healthy group (P<0.05), the expression levels of COX-2 mRNA [(1.68±0.48), (2.37±0.54) vs. (1.01±0.34)], RORγt mRNA [(1.72±0.57), (2.44±0.81) vs. (1.05±0.35)], the percentage of Th17 cells [(3.87±1.29)%, (6.71±2.34)% vs. (1.99±0.66) %] and Th17/Treg [(0.63±0.21), (2.46±0.82) vs. (0.23±0.08)] were higher than those in the healthy group (P<0.05); the expression levels of serum miR-143-3p and Foxp3 mRNA and the percentage of Treg cells in the SAP group were lower than those in the MAP group (P<0.05), the expression levels of COX-2 mRNA, RORγt mRNA, the percentage of Th17 cells and Th17/Treg were higher than those in the MAP group (P<0.05); the expression level of serum miR-143-3p in SAP patients was negatively correlated with Th17, RORγt mRNA, and Th17/Treg, and the expression level of serum COX-2 mRNA was negatively correlated with Treg and Foxp3 mRNA (P<0.05), the expression level of serum miR-143-3p in SAP patients was positively correlated with Treg and Foxp3 mRNA, and the expression level of serum COX-2 mRNA was positively correlated with Th17, RORγt mRNA, and Th17/Treg (P<0.05), the expression level of serum miR-143-3p in SAP patients was negatively correlated with COX-2 mRNA (P<0.05); miR-143-3p was a protective factor that affected the occurrence of SAP (P<0.05), and COX-2 mRNA and Th17/Treg were risk factors that affected the occurrence of SAP (P<0.05).Conclusion The expression level of serum miR-143-3p in SAP patients is low, and the expression level of COX-2 mRNA is high, both of which are related to the balance of Th17/Treg miR143-3p may be a new target for the treatment of SAP. |
