| 目的评价血管内弹簧圈介入栓塞治疗脾动脉瘤的疗效。方法回顾性分析 2017年 10月至 2021年 5月皖南医学院弋矶山医院血管外科收治的脾动脉瘤病人 11例的临床资料，通过股动脉穿刺经微导管将可控弹簧圈( Boston Scientifc interlock)单纯瘤体填塞 6例，行瘤体、入瘤动脉及流出道填塞 2例，行瘤体及流出道填塞 3例。结果术中出现脾动脉夹层 1例，经脾动脉中段弹簧圈栓塞后夹层消失。其余 10例经弹簧圈栓塞的脾动脉瘤病人术后影像学效果满意，术后出现右下肢腘静脉血栓 1例，经给予抗凝治疗 4d后超声复查示腘静脉通畅。术中、术后未出现动脉瘤破裂出血，未出现手术穿刺区域相关的并发症，随访所有病例未见瘤体进一步增大或复发。结论弹簧圈介入栓塞治疗脾动脉瘤具有微创、安全、有效的特点，是治疗脾动脉瘤的有效方法。 |

| Objective To evaluate the efficacy of intravascular coil interventional embolization in the treatment of splenic aneurysms.Methods The clinical data of 11 patients with splenic aneurysms treated in the department of vascular surgery of Yijishan Hospital of Wannan Medical College from October 2017 to May 2021 were retrospectively analyzed. Through puncture of femoral arteryand microcatheter, the simple tumor body of Boston Scientific interlock was filled in 6 cases, the tumor body, aneurysmal artery and outflow tract were filled in 2 cases, and the tumor body and outflow tract were filled in 3 cases.Results Splenic artery dissection occurredin 1 case. The dissection disappeared after embolization with coil in the middle of splenic artery. The other 10 patients with splenic aneurysms embolized by coil had satisfactory postoperative imaging results. One case had popliteal vein thrombosis of the right lowerlimb. After anticoagulant treatment for 4 days, ultrasound reexamination showed that the popliteal vein was unobstructed. There was noaneurysm rupture and bleeding during and after the operation, and there were no complications related to the puncture area. No furtherenlargement or recurrence of the tumor was found in all cases.Conclusions Coil nterventional embolization for splenic aneurysms isminimally invasive, safe and effective. It is an effective method for the treatment of splenic aneurysms. |
