      目的探讨微 RNA-582-5p(miR-582-5p)和音猬因子( SHH)在结直肠癌病人病理诊断和预后评估中的价值。方法选择 2017年 6月至 2018年 12月沧州市人民医院收治的 125例结直肠癌病人及 97例结直肠息肉病人作为观察对象,收集癌旁组织、结直肠息肉组织、结直肠癌组织标本,实时萤光定量逆转录聚合酶链式反应( qRT-PCR)检测各标本中 miR-582-5p、SHH mRNA表达水平,免疫组织化学法检测 SHH蛋白表达, Pearson法分析结直肠癌组织中 miR-582-5p与 SHH mRNA表达水平的相关性,双萤光素酶报告基因实验验证 miR-582-5p和 SHH的靶向关系,受试者操作特征( ROC)曲线分析 miR-582-5p、SHH mRNA对结直肠癌的诊断价值。结果在癌旁组织、结直肠息肉组织、结直肠癌组织中 miR-582-5p表达水平[( 1.05±0.15)、(0.83±0.12)、(0.65±0.09)]依次降低( P<0.05)SHH mRNA表达水平[( 1.03±0.12)、(1.78±0.34)、(2.47±0.41)]及蛋白阳性表达率依次升高( P<0.05);相关性分析显示,结直肠癌,组织中 miR-582-5p与 SHH mRNA表达呈负相关性( r=?0.63,P<0.05),双萤光素酶报告基因实验结果证实 miR-582-5p和 SHH存在靶向关系; miR-582-5p在肿瘤分期 Ⅲ~Ⅳ期、有脉管侵犯和有淋巴转移的结直肠癌组织中表达水平显著低于肿瘤分期 Ⅰ~Ⅱ期、无脉管侵犯和无淋巴转移结直肠癌组织中的表达水平( P<0.05),SHH在肿瘤分期 Ⅲ~Ⅳ期、有脉管侵犯和有淋巴转移的结直肠癌组织中阳性表达率显著高于于肿瘤分期 Ⅰ~Ⅱ期、无脉管侵犯和无淋巴转移结直肠癌组织中的阳性表达率( P<0.05); miR-582-5p、SHH mRNA联合诊断结直肠癌的 AUC为 0.92,95%CI:(0.88,0.95),P<0.05,灵敏度为 86.40%,特异度 85.57%;miR-582-5p低表达和 SHH阳性表达病人无进展生存期和总生存期显著低于 miR-582-5p高表达和 SHH阴性表达病人( P<0.05)。结论结直肠癌组织中 miR-582-5p低表达、 SHH高表达,且与结直肠癌病人临床病理特征和预后有关,对结直肠癌的病理诊断和预后评估具有一定价值意义。

      Objective To investigate the value of microRNA-582-5p (miR-582-5p) and sonic hedgehog (SHH) in the pathological diagnosis and prognosis evaluation of colorectal cancer patients.Methods A total of 125 patients with colorectal cancer and 97 patientswith colorectal polyps who were treated in Cangzhou People′s Hospital from June 2017 to December 2018 were regarded as observationobjects. The paracancerous tissue, colorectal polyp tissue, and colorectal cancer tissue specimens were collected, the expression levelsof miR-582-5p and SHH mRNA in each sample were detected by qRT-PCR, the expression of SHH protein was detected by immunohistochemistry, the correlation between miR-582-5p and SHH mRNA expression in colorectal cancer tissues was analyzed by Pearson method, Dual-luciferase reporter gene experiment was conducted to verify the targeting relationship between miR-582-5p and SHH, the diagnostic value of miR-582-5p and SHH mRNA in colorectal cancer was analyzed by ROC curve. Results The expression level [(1.05±0.15), (0.83±0.12), (0.65±0.09) ] of miR-582-5p in adjacent tissues, colorectal polyp tissues,and colorectal cancer tissues decreased sequentially (P<0.05), while the SHH mRNA expression level [(1.03±0.12),(1.78±0.34), (2.47±0.41) ] and protein positive expression rate increased sequentially (P<0.05); correlation analysis showed that miR-582-5p was negatively correlated with SHH mRNA expression in colorectal cancer tissues (r=?0.63, P<0.05), The results of the dual-luciferase reporter gene assay confirmed that there is a targeting relationship between miR-582-5p and SHH; the expression level of miR-582-5p in colorectal cancer tissues with tumor stag es Ⅲ-Ⅳ, with vascular invasion and lymphatic metastasis was significantly lower than that in colorectal cancer tissues with tumor stages Ⅰ-Ⅱ, no vascular invasion and no lymphatic metastasis (P<0.05), the positive expression rate of SHH in colorectal cancer tissues with tumor stages Ⅲ-Ⅳ, with vascular invasion and lymphatic metastasis was significantly lower than that in colorectal cancer tissues with tumor stages Ⅰ-Ⅱ, no vascular invasion and no lymphatic metastasis (P<0.05); the AUC of miR-582-5p and SHH mRNA combined diagnosis of colorectal cancer was 0.92, 95%CI:(0.88,0.95), P<0.05, the sensitivity was 86.40%, and the specificity was 85.57 %; The progression-free survival and overall survival of patients with low expression of miR-582-5p and positive SHH expression were significantly lower than those of patients with high expression of miR-582-5p and negative expression of SHH (P<0.05). Conclusions The low expression of miR-582-5p and high expression of SHH in colorectal cancer tissues are related to the clinicopathological characteristics and prognosis of patients with colorectal cancer, which has certain value for the pathological diagnosis and prognosis evaluation of colorectal cancer.
