| 目的探讨阴道结节性恶性黑色素瘤( VNMM)液基细胞学病理，阐述黏膜型恶性黑色素瘤的发病机制并总结其形态特征，为无创性术前诊断提供理论依据。方法回顾性分析广州市番禺区妇幼保健院病理科于 2021年 11月收集的 1例被诊断为 VNMM的临床资料、液基细胞学涂片、免疫细胞化学及组织学对照结果并复习相关文献。结果 VNMM病人，女性， 52岁，阴道镜下见蓝色结节状肿物。液基细胞学见大量单个散在的肿瘤细胞，成团的肿瘤细胞团排列松散。特征性的细胞形态包括横纹肌样瘤细胞、双核及多核肿瘤细胞、八字状核细胞、核内包涵体及大而明显的核仁。部分肿瘤细胞胞质内散在黑色素颗粒。结论阴道恶性黑色素瘤可以结合蓝色的外观、液基细胞学形态特征及免疫细胞化学结果明确诊断，为临床提供无创性术前诊断理论依据。 |

| Objective To investigate the case of vaginal nodular malignant melanoma (VNMM) with liquid-based cytology, elaboratethe pathogenesis of mucosal malignant melanoma and summarize its morphological characteristics, so as to provide a theoretical basisfor noninvasive preoperative diagnosis.Methods The clinical data, liquid-based cytological smears, immunocytochemistry and histological control results of a case diagnosed with VNMM collected by the pathology department of Guangzhou Panyu maternal and childhealth hospital in November 2021 were analyzed retrospectively, and the relevant literature was reviewed.Results VNMM patient, female, 52 years old, showed blue nodular mass under colposcopy. Liquid based cytology showed a large number of single scattered tumorcells, and the clustered tumor cells were loosely arranged. The characteristic cell morphology included rhabdoid tumor cells, binuclearand polynuclear tumor cells, splayed nuclear cells, intranuclear inclusions and large and obvious nucleoli. Melanin granules were scattered in the cytoplasm of some tumor cells.Conclusion Vaginal malignant melanoma can be clearly diagnosed by combining the blue appearance, liquid-based cytological morphological characteristics and immunocytochemical results, so as to provide a theoretical basis for clinical noninvasive preoperative diagnosis. |
