      目的探讨冠心病合并冠状动脉钙化( CAC)的危险因素,构建并评估个体化预测冠心病合并 CAC发生风险的列线图模型。方法选取 2019年 1月至 2021年 6月河北中石油中心医院收治的 390例冠心病病人为研究对象,根据病人是否合并 CAC分为 CAC组( n=305例)和非 CAC组( n=85例)。采用 logistic回归模型,分析冠心病合并 CAC的独立危险因素。应用 R语言软件( R 3.6.3)构建列线图模型,并对该模型进行验证。结果单因素分析显示,非 CAC组和 CAC组糖尿病、年龄、身体质量指数( BMI)、高脂血症、低密度脂蛋白胆固醇( LDL-C)[( 2.14±0.35)mmol/L比( 2.47±0.39)mmol/L]、血糖[(5.16±1.37)mmol/L比

      Objective To explore the risk factors for coronary heart disease (CHD) complicated with coronary artery calcification(CAC), and to establish and evaluate an individualized nomogram model for predicting the risk of CHD complicated with CAC.Meth. ods A total of 390 CHD patients admitted to Hebei CNPC Central Hospital from January 2019 to June 2021 were selected as the re.search subjects, and the patients were divided into the CAC group (n=305 cases) and the non-CAC group (n=85 cases) according towhether they had CAC. A logistic regression model was used to analyze the independent risk factors for CHD complicated with CAC. Rsoftware (R 3.6.3) was used to construct a nomogram model, and the model was verified.Results Univariate analysis revealed that the non-CAC and CAC groups showed that diabetes, age, body mass index (BMI), hyperlipidemia, low-density lipoprotein cholesterol (LDL-C) [(2.14±0.35) mmol/L vs. (2.47±0.39) mmol/L], blood glucose [(5.16±1.37) mmol/L vs. (5.52±1.05) mmol/L], glycosylated hemoglobin (HbA1c) [(5.58±0.83) % vs. (5.94±1.30) %], and hypertension were associated with the occurrence of CAC (P < 0.05). Logistic regres. sion analysis showed that LDL-C, age, diabetes, hypertension and hyperlipidemia were independent risk factors for CHD complicated with CAC (P < 0.05). Based on this, a nomogram model for predicting CHD combined with CAC was established, and the model valida.tion results showed that the calibration curve was a straight line with a slope close to 1. The Hosmer.Lemeshow goodness-of-fit test was χ2=7.22, P = 0.513, and the area under the ROC curve was 0.85.Conclusion The nomogram prediction model constructed on the ba. sis of age, hypertension, diabetes, hyperlipidemia, and LDL-C, which are five independent influences on the occurrence of CHD com.bined with CAC, showed good differentiation and consistency.
