| 目的探讨肾血管平滑肌脂肪瘤合并肾静脉瘤栓行肾部分切除术加瘤栓取出术的治疗效果。方法总结合肥市滨湖医院 2021年 10月收治的 1例肾血管平滑肌脂肪瘤合并肾静脉瘤栓病人的手术治疗方案。结果病人术前行腹部增强磁共振加三维重建提示左肾占位(大小约 51 mm×36 mm×55 mm)、左肾静脉瘤栓，考虑血管平滑肌脂肪瘤。于 2021年 10月 21日行左肾部分切除术 +肾静脉瘤栓取出术，病人术后第 1天复查肾功能肌酐提示 113 μmol/L，术后 4d拔出肾周引流管， 1周后拆线出院。病理结果:左肾肿瘤及肾静脉瘤栓均为血管平滑肌脂肪瘤。术后 3个月复查肾功能提示 69 μmol/L。结论合并静脉瘤栓的肾错构瘤病人通过个体化治疗，制定详细手术方案，可行保肾手术，病人受益更大。 |

| Objective To investigate the therapeutic effect of partial nephrectomy with tumor thrombus removal for renal angiomyoli.pomam combined with renal vein tumor thrombus. Methods The surgical treatment plan of a patient with renal angiomyolipomamcombined with renal vein thrombus admitted to Binhu Hospital of Hefei in October 2021 was summarized.Results The patient under. went preoperative abdominal enhancement magnetic resonance and three-dimensional reconstruction, which suggest left renal tumor(size about 51 mm×36 mm×55 mm) and left renal vein tumor thrombus, and renal angiomyolipomam was considered. On October 21,2021, a partialnephrectomy of the left kidney and removal of renal vein tumor thrombus was performed. The patient's renal function cre.atinine was retested on the first day after surgery 113 μmol/L. The perirenal drainage tube was removed 4 days after surgery. the stitch.es were removed and the patient was discharged 1 week later.Pathological results:both the left kidney tumor and renal vein tumor throm.bus were angiomyolipomam. The renal function was reviewed 3 months after surgery 69 μmol/L.Conclusion Patients with renal angio.myolipomam combined with venous tumor thrombus benefit more from partial nephrectomy through individualized treatment and de.tailed surgical plans. |
